Both Altis (Limnos in RL) and Stratis (Agios Efstratios, but colloquially Ai Stratis in RL) are real islands. Bohemia designed them to be 1:1 geographically and topographically.
The only part that isn’t real, is the man made peninsula airbase on Stratis. This island was made sometime in the fictional future by the military in the game.
