Change the display mode. (borderless, full screen)
Press "Apply" next to resolution.
Open the graphics drivers control application. Make sure the settings for Koikatsu doesn't disable Anti Aliasing.
The bigger breasts are - the more these polygon boundaries are visible (they can be seen at some other body parts as well).
Tesselation shader can reduce this problem.

You need to install KKUTS(KoiKatu Unity-chan Toon Shader) 2.0 or older to be able to use tesselation.
You will need to apply this shader manually on each character in character editor though (kkuts manual contains more details).
Google "KKUTS(KoiKatu Unity-chan Toon Shader)_v2.3" to get it if you are still interested.
tl;dr: It's possible to make it's smooth, but it'll requre some hassle.

It must certainly require some other modifications and research to find the right settings, you're right it must take some hassle.
Anti Aliasing works with and without mods. I am using 3840x2160, too.
So: Maybe you have a graphics plugin installed which disable anti aliasing?
Try to disable all plugins first. To do that, rename the winhttp.dll in your gamefolder to -winhttp.dll or move it out of the game folder. Then start the game and Character editor to see if anti aliasing works now.
On your computer the outline of your character looks better than on my screen ? Because I don't realize if there is a big difference or not.
It's especially around the naked neck that we can see that it's not smooth I think.

On your last picture here: https://i.imgur.com/tAyYu5P.png I also noticed the same non-smooth contours as in my screen.
I don't know but maybe there's nothing we can do about it and it's normal.
This picture illustrates anti-aliasing, which makes lines itself less jaggy.
Non-smooth contours are caused by model having not too much polygons itself, so it has nothing to do with antialiasling.
Tessealtion is necessary to fix this.
Btw on the picture demonstrating tesselation from KKUTS creator himself (which I posted above) it clearly don't affect texture brightness, so there is surely some way to apply it properly.
So there's something I didn't set up correctly, I'll do some more tests, thanks.
