Having the same Salvatore in every settlement as the starting point is immersion breaking; he should have been in only one location, with different NPCs at other cauldrons.
Having Halloween costumes locked behind obtaining boss drop ichor prevented low level players from accessing the trick or treat baskets. And even then, the candy is gated to level 60 characters. Do you know what Halloween is all about? Dressing up in a costume and going Trick or Treating and getting candy.
Trick or treating should be accessible to everyone from the beginning with low level treats that provide sometimes useful buffs, sometimes funny effects, sometimes even debuffs that last like 15 to 30 minutes. That's the whole "trick" part of Halloween.
The boss fight mechanic is frustrating. You MUST bind to a camp right next to the fight otherwise you will lose out on any progress and even though you were there in the whole fight, you won't get the ichor drop if you die and are too far away to get back in time. The time to return to the fight needs to be extended by at least 10 minutes.
Having the boss icon appear on the world map in different regions at the same time is confusing. There is no indication where he will turn up next. A world message "Baalphazu has been spotted in _______! (Fill in the blank), would at least let everyone know where to go.
The costume transformations are nice. I liked looking like the different NPCs like Thorpe, but there should have been more than four of them. Typical attire like a witch, a skeleton, ghost, or zombie were missing. The weapon and armor patterns, emotes, and furniture are all nice to get which is the only reason to participate. And that's only with a high level character.
All in all, 5/10. It needs some changes to make it better for next year.
EDIT; As mentioned in a post below, you can actually get in on the boss fight at a low level but will die a lot. There are some waves that emanate out from Baal that severely injure high level characters and one-shot lowbies. Right now it's really not worth joining in unless you are at least level 60.
ex. You Have Wind/tornado psshing Player, but its only used once in MSQ at Greats Cleave.
You can participate in the boss fight even as low as 25. But, you will die. A lot. Bind to a camp next to the fight and as soon as you die, respawn and get back into the fight. You should get the ichor and can turn it in for tokens which you can use to buy one of the three lowest level items. Since you can't start the quest until you are level 35 you won't be able to access the higher tier items. But at least you can get the Trick or Treat basket for your house and a couple of low level patterns.
You may get candy but won't be able to eat it as it is level 60 consumable.
