Incase you don't know what the DLC does here is a short list:
- mutated expiditions (dungeon)
- raids
- elite trials
- daily soul trials (a harder version of an already ingame solo boss fight)
- new area
- flail and shield (weapon)
- max gear score 700 or 725 (not sure actually)
- max lvl 65
- heartrunes (like an ultimate ability)
- artifacts (special very strong armor pieces or weapons)
- mounts
- season trial (like the soul trials but for up to 10 players and only two selected ones per season)
So as you can see it brings much endgame content but before the endgame it won't make a real difference. Just try the game and decide later about the dlc.

I'm also a returned player and I love it (now) - It plays more smoothly somehow. The changes are more detailed, so you don't have to worry about getting lost. In contrast to Throne and Liberty, I can immerse myself in the world much better here and I really like the combat system now.
You have nothing to lose, take a look. The addon is completely irrelevant at first glance and only becomes interesting from level 25 (mount) at the earliest. So, apart from a little time, taking another look shouldn't cost you anything.
