Index: No Audio - Failed to play test tone
Audio worked once after a computer restart, now it won't play sound and gives ^ this error when testing audio in the sound panel. Tried ejecting USB and re-plugging in/restarts several times. Doesn't work.

Had the same issue; the device was there and set as the default, but no sound and that same error when testing. Scanning for changes in Device Manager resolved this for me.

Try unplugging the break-away box and re-plugging it. That usually works for me.

my solution was using old Vive Link Box beacuse it has its own power supply and maybe your motherboard cant push more POWER in those usb connection.

I'm curious VikingAlex, how do you connect it to the linkbox? I don't have a male connector coming from it.

I had the same problem: I'm on Windows 10 1903, with a GTX 1080, and the Index HMD audio device appeared, but wouldn't produce sound. Right clicking on it in the sound control panel and selecting "Test" prompted a dialog box of "Failed to play test tone."

I don't think scanning for changes in the device manager was part of the solution for me, but I did try it. I think what ended up making the difference was installing the NVIDIA HD audio drivers, then re-enabling the Index HMD device, which had disabled itself upon driver installation. My guess is this is because the HMD audio is going over DisplayPort, which lacked audio drivers?
