I do not know of all of the instances where it happened, but "Noble Coat" was giving a Female variant and "Gothic Maid" was given a Male variant.
Search up "Arks-Layer" or "PSO2 Tweaker" for the translation of the Japanese version.
Also make sure to go to Ship 2 to find other English players (I didn't realize that until after I already made a character).
It is basically a trade between having the full game without censorship at the cost of having less English-speaking players to play with.
Not as far as I can recall, the problem is that specific clothes like the one you want, or all sorts of other things are not included in the files (it doesn't appear to use all of the assets of the JP version if I remember correctly, also parts of the client are older than the JP version) and the game will usually check the hash of the files using nProtect Game Guard and will try and either close the game or disable the file that has been modified.
Although it is possible to mod clothes by tricking Game Guard, it is difficult to add new clothes or other things.
(Mods do exist but they mostly are just color changes of weapons or texture edits)
Also the other elements of censorship are even harder to mod, stuff like scenes which were removed are hard to mod in and reduced maximum height and increased minimum height are also hard to remove (both are minor enough that most users just use the full JP version).
The good thing is that if someone does manage to mod everything in, SEGA doesn't seem to care about mods unless it changes game play or something else.
Basically, the only way to get the uncensored game is to make a whole new character (the character appearance can be saved as a file though and copied over to the full version, assuming you clicked save on the Creation Data* while in the Fashion Room*, you can just copy-paste the file).
*It might be called something different in the NA version.
The main difference between the English and the full Japanese is mainly that:
2. More weapon camos, some are missing in NA because of licensing reasons.
3. Easier to get SG as it basically throws them out like candy.
4. More story content (EP 6 instead of EP 4)*
5. (May be outdated) Classes that aren't in NA like Hero, Phantom, and Etoile.*
6. (Also from google searches) No Mesata bots (I didn't actually know they existed until looking at r/PSO2)
7. (May be outdated) Crafting weapons*
8. Old-Type weapons and 15 star weapons
9. Higher level class and subclass cap (95 max in JP)*
10. (May be outdated) Complex PA and Dark Blasts*
11. (May be outdated) Extra skills that are missing in NA*
12. Several quests based on EP 5-6 content.
13. (Could be outdated) Treasure Shop
14. Smart Inner Innerwear default option (unintrusive innerwear)
15. Minimum height limit of 140cm (4.59ft) instead of 153cm (5.01ft) and it seems like the maximum height is a bit weird (Auxiliaries are 3ft minimum I think)
16. The original forms of some clothes (minor changes in NA)
17. Flavor text on weapons and units that is changed or not present in NA
18. (assuming PSO2 Tweaker) Better English translations of basically everything (The translation company is good, just that they probably are focused on PSO2 NGS and forgot about PSO2NA, also SEGA is losing more money than anticipated on financial reports, also Sega of America was basically non existant for a while)
19. Better customer service (but only with OTP enabled)
20. Less lewd Symbol Arts (mods seem to be more active)

2. Fresh Finds Shop (probably a replacement for Treasure Shop).
3. Players that you can actually talk to in English (Unless you are in ship 2, 4, or 10, you can't find anyone to talk to).
4. Much less lag it seems (in JP, markers and players jump around like crazy due to a 0.3s latency).
5. Voice chat (Xbox).
6. Simpler login (No need for constantly opening your password manager to enter the password in).
7. Easier way to pay for stuff (For JP, you either need a Credit Card or download PSO2es and use Google Play Services).
8. NA exclusive accessories and clothes (probably to make up for the lack of licensed fashion).
9. You don't need to use weird workarounds for the captcha as you don't need one with Steam.
*Pretty sure they would add these later on, the NA client still has the same bugfixes as the JP client and probably has parts of the same content.
Thankfully, as long as they don't have separate regional servers for PSO2 NGS, you probably could get the full content in 2021.
TL;DR: It is kinda Microsoft who saved PSO2 NA with truck loads of money and Azure servers, and also Microsoft who probably asked for the annoying changes.

1/2 right. You can find MTX/Fasion like that on the JP version, but not because it's the "uncensored" version lmao. Only because there are more on the JP version.
