99% items and 99% map?!?!?

I have been trying to track down the last bits of stuff I need to get the good ending but, I cannot seem to find what I need. I have the map upgrade and there are no grayed out areas on the map. How am I supposed to figure out what I am missing?

I am in the same position. The map seems to be fully explored, I cannot find any spot that's not explored

The only thing I can think of doing is cross referencing the full map to see if there is anything that I missed.

I found my last missing unexplored room. For me it was a room with a door opening in the map above a save room. It's probably not same for you.
I don't think it was even shown in the full map at all

Going through this process and not having a good time. Worst of it is, I got 100% map clearance in Monster mode (naturally)--BUT the maps between games are identical, so it has to be a locked door somewhere.

None of the compass items in inventory help people at 99/99... which kind of stinks and feels like an oversight!
