Chapter 2 bug: Norah only writes one of the three eyes in the journal

I interacted with each of the eyes, but only one of three (the oozing one, if it matters) got noted. This made that particular puzzle unsolveable except by total chance, since I didn't know if this involved guessing the right eye, combining one or more, etc. I was able to input the solution after looking it up, and moved on to the next thing, so I don't seem to be soft locked or anything -- just a bad bug.
