Only way to get it, is to finish whole game on classic without chancing difficulty.
Even there is chapter selection, it doesn't keep record what is the hardest difficulty that chapter is done, only if the difficulty have been change.
But generally, games should be rewarding players' participation in their extra content, such as collectibles, side quests, and especially time-consuming and difficult ones.
Mafia is known for it's story and cinematic experience, hence the distinction between "Free Ride" and "Story" chapters. So I definitely think they should expect re-runs of chapters and reward players who participate in the grind.
