If so, there's only ~3 quests where you have a "follower". Of those, you are the follower most of the time.
Exceptions are inside the Brigadier Hall (when it says to explore), and on Eamonn's Isle.
I don't remember how it is for the "Mystic Hammer" and "Mountain Prison" quests atm.
(I'm currently at the "Mystic Hammer" quest in a second play-through).
I guess KOA is in good company in that regard. Skyrim, Oblivion, Sacred etc all had
occations where an NPC would get stuck because of failed pathing (or clipping).
It is a bit different from the "every quest with a follower" argument, which in my opinion
was too broad. But yes, KOA isn't updated anymore, so you can't hope or beg for
an official fix for pathing issues or anything else.
However, you can load an earlier save (if needed) and stay a bit closer to the NPC, and
try to move differently when you're close to where it stuck earlier. Hopefully you avoid the same.
Btw, the current quest-note (blue text below quest title) also tend to say/hint whether
you are the follower or the leader.
