If it's while you are actively in control of your character... The game seems to like constantly changing your gear to the default...

Also, no, the actual visualization screen has nothing to do with it. Actually, its unfortunately the only fix for the bug I can find. During gameplay itself, the game is literally forcing Kirito to wear a specific armor visually, despite what he has equipped.
Unless the devs were incredibly lazy (then why create armor models at all), I shouldn't be FORCED to use the visualization screen to get the armor I have equipped to show up visually on Kirito.
Unfortunately, the visualization doesn't fix every outfit since not all of them appear to be available for use. The Leather Armor is a good example of this. I can equip it all I want, but visually the game forces Kirito to wear the armory outfit. If I wanted him to wear the senior outfit for example, and have it show that he's wearing it, I have to equip it THEN go into the visualization screen and force the game to show the senior outfit.
Its ridiculous.
I've already run the file check twice and filed a complaint through Bandai's tech support. I even tried forcing a visualization for one of the DLC armors, changing areas, then the armor I was wearing, then removing the visualization and it still shows the armory outfit from the above screenshots. Just have to wait to see what they say. I'm going to be more than just a little annoyed though If I have to start all over yet again thanks to a major bug. That tutorial is too dang long...even skipping all the cutscenes...
I'm going to try uninstalling and reinstalling the game when I head off to bed and hope that fixes it (and doesn't remove my saves in the process ><)...
Visualization of course overrides the standard armors appearance, but the standard armor equipment should change visually for each 'class' or 'type' of armor, not every piece is unique though - at least according to my experience prior to today....
After initially posting, I tested it out in game again.... On the protagonist, every armor is visually as you said - the Armory Clothes.
I tested on Alice a bit too, all of her Visualization armor made her disappear, with the exception of the black robe.
Don't know if some progression point or random bug triggers it in the game, or if it has to do with settings.... My main mission is searching the map after the desert. I just changed a few settings in nVidia Control Panel, like forcing V-sync Off, and setting my refresh rate to Samsung - Preferred...
Either way, something is bugged.

Edit: Actually... That may be as they designed it.... The armor equipment default appearance is whatever the last relevant piece of armor Kirito acquired in the story...
I realized I don't really have enough data to say... The only time I really changed my equipped armor was from the armor that was currently default in the story to Blacklight Clothes. I also mainly kept the initial clothes on as Visualization. And since I already knew what the previous armors looked like and what their stats were, I didn't bother to ever equip them again.
I did notice the other day that the Armory Clothes and the Blacklight Clothes were identical, but thought it was similar to Hollow Realization, like I mentioned before the edit....

It very well may be a canned response...we'll see...
And for the record, uninstalling and reinstalling did nothing to address the issue... It also doesn't explain though why some armors don't unlock a visualization more (like aforementioned leather armor).
I looked at every outfit my characters have right now and they all have a 3d model icon with a neck.
Yes, it developer being lazy and trying profit with the cube because you won't spent money if great skins and armor already exist in the game so enjoy gacha of salt.
The visualization menu was ALWAYS only ever advertised as the solution for those ever common cases where you found better armor than you had, but did not like the appearance, and thus wanted to use your older armor's appearance (ex. Kirito all of first season only ever using one suit of armor that he received from the f;ppr one boss).
