Fortunately, Serana always seems to show up, near the end of a task if not before. I admire her independence, but sometimes she can be a bit exasperating in that respect. Once she simply decided to wait near the entrance of a cave and let me fight whatever I was fighting in that cave on my own. It wasn't a very large cave, and I completed that task relatively quickly. I exited the way I'd entered, and she and I met up on the way out of the cave.
When I was gathering Spriggan sap for 'Healing A House' in Solstheim, I finished killing the last Spriggan. When I looked around for Serana, I couldn't find her anywhere nearby (I even zoomed out to see if I could locate her and couldn't) and sadly was afraid I'd lost her. I decided to fast-travel back to the Headwaters to soak the stuff, complete the quest, then go look for her. Happily, she was outside the Headwaters waterfall waiting for me.
As Trigger noted, followers who are still following you will usually be nearby after you fast-travel. If you ask them to wait and then they get tired of waiting and leave (a follower before Sarena joined me did that), I'm not sure where to go looking for them.
I think that companions that get tired of waiting at a location you tell them to wait go back to the original place you find them. For example, the inn or town you found them in.
If you haven't told them to wait somewhere, then fast travel to the nearest place and they will appear. If not, fast travel again until they do.....they will.
Unless you told them to wait somewhere and you forgotten you did that. In which case, you're gonna have to remember, .....or find them systematically by hand....
