The most obvious reason would that you also mapped the special attack to 'left', but since you emphatically mention you checked that, I guess that can't be it :P
Just as an experiment, you can try to map 'left' to another button. If the problem still occurs, you have something very weird going on. You can look for the configuration tool for your controller, may that will provide some answers.
Unfortunately no. You can tell when rep goes up by the heart that popups up near the character's heart during that transaction, but you still have to track the number of hearts manually. Yes, that sucks.
I only use the D-Pad/Analog when mapping Up?/Right?/Down?/Left? after that comes the Attack/Confirm and lastly Special/Cancel.
So I highly doubt I used left for special attack.
