Nightmare is a weird difficulty. They made it mind blowingly difficult to even get your foot in the door, a part of it being attributed to the fact that you don't even start seeing mythics all that often until chapter 2 or 3. Chapter 1 is Nightmare difficulty but will not give you equipment to do Nightmare. This is incredibly odd.
I eventually farmed up a great Pawn Shot on Kings Game Insane and upgraded it as much as possible. I then took it to Endless Spires Nightmare. I used EV barriers, gas traps, and towers to deal with the trash (towers/traps actually get a buff in Nightmare), and used the pawn shot to wittle the ogre down. After the first wave, I looted all the chests and left to do it again. I got anywhere from 1-3 mythics per run. I needed a combined effort of 3-4 'godly' geared characters to actually have a chance of getting mythics, myself.
This was. . slow. And time consuming.
The best advice I can give is get help from someone geared to do Nightmare. This is simply, by design, the easiest way to dive into Nightmare. Otherwise it's a steep uphill climb to even be capable to do the first couple of chapters. I'd be willing to help a bit, though I've been very busy with work stuff as of late. I'm at a point where I can do survival nightmare in most of the chapters, and it literally rains mythics past wave 10.
Past that, I dunno. I've bought all the transcendant gear I have. Those average 300m+ a piece.
