Microphone Manners
Does anyone else get bothered when someone has NO respect for microphone manners? I can understand wanting an open mic and an open flow of communication, but don't have your TV blasting in the background, or music, or five million children and family members... If you're going to play co-op with an open mic, please have the courtesy to be in a quiet spot, or make it so you have to push to talk.

I hate having to disconnect from someone because they refuse to turn their mic off in a noisy environment, or because they have no respect for me having to listen in to everything they do/say. I can understand a bit of background noise, but when I can't properly concentrate? No.

Anyone else deal with this, (is it widespread?) or am I dealing with the rare few that don't understand how things work?

It's very widespread among children. L4D2 will be full of adult voices, Spiral Knights will be full of television and mumbling.
When they refuse to turn their mic off, I would insult them and leave.
Eventually, negative reinforcement works. That's why you hardly see multi-part RARs anymore.
