As for the specific scene in the trailer, I'm not even quite sure of the situation. I have a feeling that this is supposed to be some sort of "near-death" visual sequence that happens when your character falls unconscious. The same might be true for the last scene in the trailer with the mimes by the fire.

The plague's effect on the brain is not responsible for all of the strange things in the game. However, the plague definitely does add its own level of weirdness, and many of the other strange things will be relevant in one way or another.
This should be the subtitle of the game, instead of that '2'.
Not a remaster, more like a reboot. A remaster is already available here on Steam, though.
