Yep, same here. i think it depends on things that might not be obvious.
Like the money escort for Charity, its not necessary (and not easy) to keep the courier alive until the goal, but if you do, theres an extra story option that might rack good or bad points (and a nice amount of serum)
Its hard to finish the mission with him alive but i think if you do, that also grants goodie points.
Also chosing to beat up or pay the pimp makes a huge difference.
The above are the only two things i did differently on my second playthrough and that got me the excellent rep in second chapter, first run i only got good.
I killed the Coin Killer in Chapter 2, beat up the pimp, beat up the guards instead of getting a false ID, beat up the drug dealer instead of intimate and accidentally siphoned serum from a dude and never lost Excellent; Hell I didn't even get a chance to get a fake ID for the bartender. I'm starting to think that maybe beating up mobs has a huge impact on reputation.
I reached second chapter being all goodie good and my reputation was neutral.
I was still neutral when i contacted the barman for id at the Tierville entrance.
Then i go and beat up the two guards at the Tierville entrance and lo and behold my reputation is good.
This game needs some fixes looks like, because this is totally wrong.
Looks like the best way to get reputation is start fights with the guards that you can dialogue with.....this is total nonsense.
