2. Probably cheaters. You can go fast when using a boost just before crossing the line (and get a boost pickup right after), but some are even faster than that, so that's pretty much cheating.
3. Try beating the expert ghost and see if the medal appears then. I haven't experienced this myself however.
2. Yes there are quite a lot of cheating players on Time Attack. In the next update the leaderboards will get reset and Valve Anti Cheat will get added. This should hopefully keep them away.
3. I don't know if you're supposed to see the medals on the track menu, but that might just be a bug. Not much to worry about.

2. yes they're cheaters, and should be punished. therefore....
this needs to happen... banning a steam account (and robbing them of ALL there associated steam library) is about as tough as it gets on a digitial delivery platform, and valve need to set the industry standard imo... it cannot be tolerated :/
i've 185 games on steam atm. and i certainly wouldn't risk losing all of that just to get good at SART... wtf :/

if you practice there's no need to let go of accelarate at all... just let go of drift (very very very briefly) whilst switching the other way...back, and forth, and back, and forth...
you can hold a lvl 3 boost round a whole lap if you try! :D
