Kids not getting named after family name?

why when i have a child
after making a custom ruler
does it not use the family name
as it's surname
and instead my first name followed by son or dohtor

Pretty sure it's a cultural thing. Common with Norse and Saxon cultures. The important thing is that when you look at the character window of your children they have the coat of arms of your dynasty.
