Good point. It could be also called "Kingdom of the Levant" or "Levantic Kingdom".
1) No Teutonic Knights in Prussia (instead, near Jerusalem/Venice/Transylvania). After the Crusades, the Knights have been wandering around Europe, until Mazovian prince Conrad asked them to take over pagan Prussian lands in his name in 1226. What if he didn't do it and Poland never lost Eastern Pommerania? What if Lithuania wasn't the only Romuva nation in 1356 scenario?
2) The feudal fragmentation of Poland never ended. The testament of Boleslav III Wrymouth divided Poland onto his sons, as seen in the map in Wiki article: https://pl.wikipedia.org/wiki/Polska_w_okresie_rozbicia_dzielnicowego
Vlad I Elbow-High didn't get the chance to take over other dukes in early 14th century.
The 2nd one would be really interesting, especially since there's a decision to form Poland in vanilla EU4. Historically, the seniorate district was ruled by the oldest Piast and ruled all the duchies. It'd be a hard mechanic, so I guess the district could own vassals in the mod.
