Do I need to add death to them or leave them unkillable?

you could make it toggle so you could switch from immortal and killable

Right now it's lame that i can't kill them like i used to so i think making another mod where you can would be the easiest and simplest way to implement it. plus if people want unkillable monsters they can just go to the version of the mod that has them as such.
