Instead of spawning with 5 Blue Flies and 1 Red fly, you start with 3 Pooters and something similiar to ???'s only friend
The Character itself is Immune to all forms of damage, in fact, the one taking damage is the controllable fly.
Some Synergies i was thinking of:
20/20: Transforms the Pooters into Super Pooters
Monstro's Lung: Transforms Pooters into Tainted Pooters
Loki's horns: Has a chance to spawn a sucker which well... works like a sucker
Ipecac: Has a chance to spawn a Spit
Ball of Tar: Has a chance to spawn an Ink
Brimstone: Has a chance to spawn a Soul Sucker
Tainted Pooters from Monstro's Lung would be OP as hell
you're not wrong but still, it would be cool tbh
Soy milk: spawn rapidly tiny flies
Almond: same but chaotic
godhead: is missing for some reason and the synergy is kinda obvious
sacred heart: hollow got now bigger flies but low amount of them and they could hit more than one enemy
spirit sword: sword of flies, similar to ragged rag mod but they can pass trough enemies
continum: a spectral fly pass trough the enemy after hit and just continue the track depending on the range. It's not homing after this

starts with one fly, this fly orbits them, it will move forward when you shoot in a direction. you cannot gain items, but when you grab an item, it becomes a fly, this fly retains the traits of that item (more health = bigger hitbox, speed = return speed, any tear effects are put on the fly)
pocket item: reclaim (instant)
with this you destroy the most recent fly and apply that affect to all flies, however that effect is diminished
birthright:
all further flies will provide a very diminished buff to all flies
