Yes, a wiki is indeed necessary. But the integration with External Item Description is already there and should work, maybe something has changed in its API since the last release, in that case I will fix it
I think if viewers could have options like custom curses for example like "more enemies spawn" "picking up coins hurt you", or maybe blessings like "break rocks by making contact with them" or "temporary ladder effect". I think making chat feel like they are advancing your run whether it's in a positive manner or negative is important, what I see is that viewers get disinterested very fast because they don't feel like it matters what they are voting for even if the events are drastic it gets repetitive very fast.
