But if you don't want to add historical disasters, you may can think about random disasters, which concern an entire region like the flooding of the North Sea coast.
I wonder if earthquakes and hurricanes could be simulated. Hurricanes, increase attrition on sea tiles, and earthquakes basically have the same effects as a plague would, but it would have repeatedly pound 1 province
