I think it is resolved? I played a new Liu Chong campaign and later when encountering Lady Du, I got the concubine prompt so I think its good. Haven't found anything since.
BTW, a question: is it supposed to work for *every* unique/legendary female character, or just for the MTU exclusive ones? Asking because, you know, getting Sun Ren with Liu Bei always is quite tricky, even with dedicated mods.
I think it only work if it's a Legendary. I was able to get Sun Ren to work in a different play through. I was even able to do so with Lady Bian after beating Cao Cao as Lu Bu

They obviously have to not be already married for the event to trigger. Not sure if it would still trigger when, for some reason, you ended up being a distant relative with one of them.
I tested it a bit more in the middle of a campaign using the Sandbox console to steal some waifus: everything worked fine, to the point I noticed you can receive multiple dilemmas in the same turn if, for any reason, you got several potential concubines at the same time. I even got to 4+ concubines while testing, so there doesn't seem to be a hard cap (but, as stated, Lady Du behaved strangely for me too, as she made me wait 25+ turns even though she didn't seem already married).
Another thing I'm still not sure how it works is about children: the game is supposed to set a max number of children (usually 4-5 tops, as each child lowers the chance to have more): if it's max per character then no matter how many concubines you get, you'll still be bound by that; if it's max per couple then perhaps the limit is bypassable with the concubine system (and the family tree might become unwieldly :P ).
If per chance you capture the [legendary] character and she's still married, you can simply have the character divorce and on the next turn the event will prompt for your character to marry her.
As far as children are concern, I once had more than 5 kids (not grown up yet). It's a ridiculous long horizontal line lol They all just shows under your character and whoever is the current concubine is though all of them come from different mothers. It's pretty insane at the late game where they all grew up and you suddenly have free generals
Could be a compatibility issue with a mod called Guanxi Overhaul, which adds events based on relationships (likes and dislikes from traits).
Edit: Just tried MTU by itself with no other mods present during Lu Bu's 194 campaign. Works for Diaochan, though I never got the event trigger for Lady Du after 10 turns, could be the latest update causing issues too.
