I think my "achilles heel" was no transportation between stores, as moving stored goods and food is somewhat different to how Banished works. As a result some villagers were suffering while others were all fine and dandy. Donkey farming and transportation was being built when I lost a huge chunk of my villagers.
So I stepped back a few saves and started construction of the donkey farm and Trade building a lot earlier, it helped but not much as I still lost villagers, but not as many.

The diet of your inhabitants shall be diversified; it will strengthen their health and immunity. If people eat only meat or only vegetables, their health will become weaker, which will lead to frequent diseases. A herb doctor collects medicinal herbs in the woods; he will cure mild diseases. And a doctor in an infirmary will help to cope with more serious diseases.
