This is a result of a pretty common bandaid that CA uses for their bad rigid models - the animators use an animation offset to move the protruding eyes backward. This is actually also fairly noticeable on Fa Zheng and Sun Ren (that offset is absurd and drastically alters the mesh). However, the animation offset is only active during battle; if you zoom in far enough on the campaign map, you'll see that Cao Pi's eyes do stick out without this mod during the turn phase. It's kinda hilarious that the CA animators received this terrible model with eyes sticking out of his face and were like "meh not my job" and slapped an offset instead of just fixing the mesh (average corporate solution). This mod actually alters the mesh but previously still had the offset as well, hence your observation.
Anyways, I've overwritten the offset so it no longer recesses twice. Should be fixed now
