I see two major possibilities for your bug, maybe they are some inactive/invisible ennemy or a trapdoor in the room, or you have a familiar that count as an ennemy, but maybe it's another thing
Does anyone have a seed with the placenta on the first floor? This way I could do some tests to detect what is causing it.

If a room without enemies summons enemies afterwards (for example, the challenge room, or battles against angels to get their key pieces), then the instant healing will also occur throughout said battle
If the author of the mod were still alive, a recommendation would be to disable the healing of this item in specific rooms
