How about 10% chance then up to 25% at 5 luck?

A possible alternative nerf is to only have it generate pills on damage and not self-damage (such as sac room spikes, blood donation).

I like this change too, but I agree with Lostfrail, 10-15% would be better, especially considering you can use this with blood donation machines.

Considering it's status as a boss room item, it should be lower imo, 10% (1 every 10 hits) would maybe be better? That way you don't rely on getting good pills and it's more a clutch moment buff. 1 pill every 5 hits is still pretty busted, especially when there's a full health in the rotation.

Either way it's a massive improvement over what it was before.
