New Game > Mods > Tick the mods you want enabled for this game
It should now work if it was ticked in the Mods menu.
I'm rather new to this game - is there a further Mod activation built into the Menus that I am missing ? Or should the Subscribe via the Workshop be sufficient ?
I'm assuming that "cant get it to work" means the vanilla names still appear and 'House {Family Surname}' doesn't. As simple as it sounds, did you activate the mod before starting a new game? Mods can only be added when initialising a new game. I have just tested the mod myself and it works fine.
