Selecting an image

Select any image at a square(1:1) aspect ratio; preferably with a resolution above 128.
Creating a .vtf file
Make a 128x128 .vtf file of your seleced image using spraygen or VTFEdit. For this guide I will be using spraygen.

1.) Import the file into spraygen
2.) Set the resolution on the bottom left to 128x128
3.) Export the .vtf manually
Placing the .vtf in the E.Y.E. folder
Place the newly created .vtf in
Steam\SteamApps\common\EYE\EYE\materials\vgui\missions
Creating a .vmt file

In order for the game to register your new icon, you must create a .vmt file.
To do this simply copy an existing .vmt file and change the included text with a program like notepad to
$basetexture" "missions/[filename of .vtf without file extension]"
Using your new icon
To get to your newly created icon go into the multiplayer tab of the options menu

Don't be shocked if you don't see it at first, to see if it works simply launch a game and type some text into the chat.

If it does not show up, check back to the previous steps in the guide.
If it does, congratulations!
