OMG! ive been looking for something like this! Is there any chance you could upload this to the nexus? Theres a bug with steam that if you reinstall windows like i did you can no longer get mods form the workshop :(

he was probably killed before the mod cause this mod makes all couriers invincible and essential so this may have just fixed it for ya! cause the same happened with me and this fixed it. hope it helps u too!

funny thing, not long after i installed this mod I was exploring a new area and I found a Courier that had been killed by trolls. Was he dead before the mod? I've never gotten the white Vial fix quest once
