Intro
I've noticed that the Revision devs left us in a dark about a lot of achievements. While some are straight-forward, have easy to understand descriptions, and can be easily achieved by simply completing all the game's missions, many others are secret, or don't have clear description text, or both. Moreover, there was no guide to getting any of those secret achievements on Steam Community, or anywhere really. The only real roadmap to getting many of these achievements was in the game's code, particularly DeusEx.u and RevisionMission.u. So I decided to make my own achievement guide, and to maintain it for as long as I keep achieving them, or at least until the Revision team decides to post a guide of their own.

Right now, this guide only covers about 1/4 of the awards in the game. So if you've earned an achievement I haven't listed, or that I've only listed in the Misc section, and it's not one you wouldn't normally get just from completing the mandatory story missions up to the three endings, please leave a comment describing which one and how you got it. If I can confirm the method works, I'll add it and credit you.

Note: I used the Biomod gameplay mode on easy to get all of the following achievements. Skill point values and item locations may differ in other game modes and difficulties.
Main Menu
1. Always Something New:
On the main menu or in Paris streets, press y or t to open the console, delete the Say or TeamSay line, and just put saxbomb. This will unlock the achievement, and a Saxophone rendition of the Revision Music. You have to have Revision music enabled to get the achievement.
Tutorial Achievements
1. All I Have is a Candy Bar
There's a hidden candy bar inside a dark pipe at the end of the intro to ladders room.

2. Don't Let It Go To Your Head
Play through the entire training level, and get to the final room with the holograms. There will be a brick extruding from a lower part of the left wall. Click it with the right mouse button, then activate the holographic messaging machine that just opened on the opposite wall to get the achievement.

3. Off to a Great Start
After you place Private Winslow on the healing bed, take him off it and carry him with you until you get to the toxic waters. Throw him in to get the achievement.
Chapter 1: The Coalition
To obtain the following achievements:
1. Small Explosive, Powerful Punch
2. Use Your Head
3. War Doctor
4. Swift as a Dolphin
Start a new game, go to the initial skill selection, select the Pistol skill and downgrade it. Then upgrade the Athletics/Swimming skill to maximum and start the game. Then start a new game and repeat the process with Medicine, Demolitions, and Environmental Training.
5. Up Close and Personal
Do the same thing as above, but even with all the initial skill points, you'll only be able to upgrade low-tech to ADVANCED, not MASTER. So without upgrading any other skills, go through the mission, sneak up on as many NSF agents as you can and whack them unconsious or dead, complete all the objectives and find all the secret areas, free Gunther and find Leo Gold, and by doing enough of these tasks you'll eventually have enough points saved up to get the final Low-Tech upgrade. I suppose you could get the other six Skill Mastery achievements by restarting the game that way with each of the other skills upgraded to ADVANCED, but it'll be even longer and harder with any of the other 6 skills.

6. How Loyal is a Hungry Turret
Sneak up to the Statue Entrance, past the guard and the bot (which I recommend taking out with the GEP gun first), and get to the login terminal. Use the Username nsf001 and the password smashthestate, and set the turrets to kill enemies. Then wait a bit for the bodies to start dropping inside and you'll have the achievement.
7. Disarmed
This one's pretty easy. Hit an enemy with enough damage from the right kind of weapons and they'll drop their firearms. Or make them follow you back to the docks and your allies will do it for you.

8. As Ordered
Get to the top of the statue, save the game, then talk to Leo Gold, and take the dialog option to capture him alive and unconscious.
9. Change of Heart
Then knock him out anyway.
10. Over the Line
Reload back to the conversation with Leo, take the dialog option to put him in a body bag, then kill him.
11. All Bark, No Bite
Attack Leo, but don't kill him, then report back to Paul at UNATCO HQ and you'll get the achievement, and a lecture.
12. No Regard For Authority
After you capture Leo, knock out or kill the soldier that shows up at the top of the statue and loot his body, and you'll get the achievement, and a brand new assault rifle.
13. Getting an Upgrade
There is an Aug canister in the room with Leo. Take it and install it using either the hidden Medbot cache on Liberty Island, or the one in Jaime's office at UNATCO HQ. Then, when you have your orders from Manderley and are ready to leave HQ, take the boat at the end of the dock where you begin your first mission to Battery Park, and when you get there you'll get the achievement.
14. Orbital Cannon
If you made it to the top of the statue without knocking out or killing at least one of the NSF thugs patrolling Harley Filbin's side of the docks, and you have a sniper rifle on you, then before you talk to Leo Gold, go to the outer area of the top of the statue. Upgrade your Rifle skill as much as you can, then point it at the docks and zoom in until one of the thugs is in your crosshairs, then shoot him until he dies and you'll get the achievement.

15. Soda Stealer
At UNATCO HQ, go to the drink vending machine next to Gunther and spend 2 credits to buy a soda. It is possible you might have to repeat the process until you get a certain flavor, but I got a blue soda and the achievement on my first try.
16. Back Door
In Alex Jacobson's office, there's a hidden panel on the floor. Find it, open it, and go down it to reveal his secret stash, and the achievement.
17. How Unprofessional.
Go to the Women's bathroom on Level 3 of UNATCO and murder the lady on the toilet. I would save just before doing this though, because I believe certain other achievements will be inaccessable in playthroughs where she's dead.
18. Iron Body
Take 300 or more total damage, then heal using a medical bot. When you're close enough to a medical bot, I recommend firing a GEP rocket or a WP Grenade just far away from yourself to harm you, but not kill you. And make sure you fire it away from the medical bot, because those things will explode after taking enough damage.

19. AM I BEING DETAINED
Knock out every single NSF member on the island except Leo Gold before you talk to him. That means not letting them get near the bots or UNATCO forces on your side, because your allies generally will hit them with lethal force if they see them. You also can't use any sort of lethal weaponry on them yourself, so only use Gas Grenades, Riot Prods, Tranq Darts, Pepper Guns, and Batons on human enemies in that level. And of course, EMP Grenades will only affect bots and electronics. You can use GEP rockets on enemy bots, just don't hit them close enough to living or unconscious enemies to kill or gib them as well. Don't let any NSF members that try to run away escape, or they could just keep running and you may never find them, or worse, they could run into UNATCO agents or bots, who will simply shoot them dead. In Shifter and BioMod, Paul makes sure to use non-lethal weaponry, so if they just run into him, they should be fine unless they shoot each other in the crossfire.

20. Friendly Fire
Get an enemy to shoot another enemy by mistake. I recommend starting the game with maxed out Athletics skill, then run towards a bunch of NSF before Paul has a chance to brief you. If he encounters them guns blazing before he talks to you, he'll have no choice but to get in a fight with them first. At least one of the times when I did this, they shot Paul so much they accidentally shot each other, and I got the achievement.
When the Revision 1.1 patch first came out, players could automatically get this achievement in the intro, when MJ12 troopers in Paris would kill each other while gunning down civilians. This was patched soon afterwards though, so it could only be achieved during the active missions of the game.

21. Emergency Extinguisher
Get yourself lit on fire, then put it out with a fire extiguisher. I recommend waiting until the end of the level, then grab the fire extiguisher behind the glass at the bottom floor of the statue near the entrance to Gunther's holding cell. Then, just outside the statue's front entrance, there's a tall cubbyhole near where the thug with the flamethrower was. There's a WP Grenade inside a fairly durable box. I recommend firing the GEP Gun at it to get it open, then take the grenade, save, and attack yourself with it. It will light you on fire, so while running away from the burned area, equip and use the extinguisher on yourself and you'll get the achievement.

22. Silence of the LAMs
To get this achievement, you have to shoot a LAM with a gun or rifle to make it explode prematurely. You can get the achievement by shooting your own LAM, and I got this by throwing one at the vending machine at the start of battery park and quickly shooting it with a Sniper Rifle, which also destroyed the vendor and uncovered the NSF's secret entrance.

23. Death from Above
Kill an enemy by jumping onto them, like a Mario Goomba stomp. I recommend maxing out Athletics, climbing up to the outer edge of the statue, finding a spot with an NSF Terrorist right below, saving, and keep trying to fall on him until you succeed.

24. Indiscriminant Surveillance
Go to the Women's bathroom on Level 3 of UNATCO and talk to the lady on the toilet before reporting to Manderley. When he finishes giving you your assignment, you'll get the achievement.

25. Violent Habit
Smoke to death; this can be done early on by smoking 10 packs at UNATCO's firing range 'till you die. Doesn't work with Shifter or BioMod.

Chapter 1 Continued
Somehow I've either been locked out from editing Chapter 1, or I've hit some kind of character limit for the section and can't add anything else without deleting some of what's already there, so I've made this additional section until I can figure out how to merge the two. Here are some more Chapter 1 Achievements:

Primary Objective Focus
Capture or kill Leo Gold in 3 minutes or less. I recommend using BioMod and maxing out Athletics; this will increase your character's speed dramatically. Then, go right from the intro at the docks before Paul even has a chance to talk to you until you hit the platform with the stacks of boxes. Climb them with the ledge-climbing feature you get from having Advanced or better Athletics in BioMod until you near the top, then run past the enemies and climb the ledges and ladders until you find Leo Gold. Blitz through the dialog with Gold, but try not to hit the first option accidentally, or else you'll have to subdue him. Then once you've captured, killed, or KO'ed him, you get the achievement.

UNATCO Order 99009: Get both arms and legs' hitpoints to 0.
Stand on a burning barrel until you zero-out both legs, then push against the barrel from your left side and then your right, and you'll achieve the 99009. [Credit to Complete Revision Achievement Guide]

Decked Out
Either attach a laser and a silencer mod to a Sniper Rifle, or a laser and a scope mod to a Mini-Crossbow or a Stealth Pistol. I got this by grabbing the Laser mod in the underground bunker, grabbing Johnathan's datacube with the login to his bank account 230023, using it on an ATM to steal his 200 credits (which together with JC's initial balance makes c700), spending the 700 on Tech Sgt. Kaplan's Scope mod, and finally applying both of the weapon mods to a mini-crossbow scavenged from an enemy.

Last Bullet, Last Chance: Kill someone using the absolute last bullet of a single type of ammo of a pistol, rifle, or shotgun. When you start at Chapter 1, max out the Weapons: Rifle skill as much as you can, then ask Paul for the Sniper Rifle, which should only have 5 bullets. Use three to waste the NSF agent in front from a far, then the last two on the one on the left on the bench. Make sure crouch down, zoom in, and hold the reticle until it's closed all the way in for as many of the shots as you can without letting the targets escape.
Chapter 2: Sunglasses On a Night Operation
1. Anarchy
Stun Prod a Riot Cop. When you take the bus out of the hostage situation in Battery Park, and end up in Hell's Kitchen, I believe if you go all the way to the right, you can view a firefight between the Cops and the NSF. Knock one unconscious with your Taser Rod and you'll get the achievement.

2. Tipsy Flying
When you get to Hell's Kitchen, clear at least 1 inventory slot, then go to the Underworld Tavern and talk to the "Pilot guy". Talk to him, buy him a beer from Shea the bartender, keep talking to him, buy him another beer, then just keep talking to him until you get the achievement. He only takes cold beer, not ones you've found laying around, so make sure to have the 80 credits ready.

3. Genetics Work
Go to Smuggler's Den under the far-right side of town, disable his security bot with an EMP, and accept Cyborg's Smuggler's rescue mission. Use his NanoKey to open the sewer hatch near the Underworld Tavern entrance, and knock out or kill all the Paramilitary Soldiers and disable the security until you find Bob Page Ford Schick. Then cover him while he escapes until he makes it to the end, alive, and when he tells you he can make it from there you'll get the achievement.

4. The Fall
I suspect this achievement is gained traditionally by dropping an unconsious body off a building to its death. However, I got it by hitting a rat at the 'Ton I was on top of with a baton until it was unconscious and then dead. It sounds more like a glitch than a sure-fire method to get the achievement, but that's how I got it, and now that I already got the achievement, it's not like I can test it again to see if the dropping body thing really works in the latest version of Revision. But I did try doing that with the initial Revision 1.1 and it didn't work, so go figure.

5. Sign Him Up
Much like the Human Revolution achievement of a similar name, it involves prolonged basketball. There's a back allyway in Hell's Kitchen where some gangsters are mugging a bum. Attack the thugs with something that won't kill 'em first, like Gas Aerosol, so they're marked hostile, because if you don't, you'll have points deducted for attacking non-enemies (I know, a screwy bug, right?). Talk to the bum, and he'll thank you for saving him with a secret password. Then you need to grab the ball laying on the court there and shoot some hoops for a few minutes until a notification pops up on the top of your HUD saying "Sign him up for the Knicks!". The achievement will also pop up.

6. Inventory Priorities
Beer, Wine, Liquor; carry at least one of each. I recommend looting the Liquor store next to the Underworld Tavern.

7. Rotten Way To Die
Knock Ford Schick out during his rescue mission, bring him to Smuggler's den, and report with him to Smuggler. JC and Smuggler will remark how he's "dead", hence the achievement. Credit to Reborn for the find.

8. Camp Fire
Set 6 people on fire at the same time. I recommend nabbing the flame thrower from the NSF guy a few floors down from the vending machine in Battery Park, then going to the shootout in Hell's Kitchen and burning as many of the UNATCO Troopers and NSF Terrorists you see as you can.

9. Hot Dog
Set fire to a dog. I recommend the above procedure, except instead of burn your buddies, go through the nearby entrance to the next map and disable the lasers, then once you're there, drop to the street level and spread the smell of napalm all over the attack dogs.

10. I Spill My Drink
Somehow, I got this by launching a GEP Rocket into the terrorists' explosives at the hostage subway.

11. Eye for an Eye
Kill someone while they're killing you. I got this one during a shootout in the subway when an NSF's stray bullet hit a TNT Barrel, killing everyone.

12. Deus Express Delivery
This one's a little tricky. On the lower level of the Generator Building, there's a break room with two boxes of pizza ("Pizza Pi - Irrationally Delicious"). Save the game, then take one to the previous map. There's a secret elevator somewhere in the adjacent building that should take you there. Be careful transporting the Pizza; it can break easily from fall damage. Once out, take Smuggler's secret password-protected elevator (bloodshot) to his lair. Once there, throw the pizza at him and you'll earn the achievement.
Chapter 3:
==UNATCO HQ==
13. Get the Hell Out of Here, Denton
Using door code 31173, enter the interrogation room of UNATCO HQ (just adjacent to the Armory), then talk to the NSF prisoner on the left until you've exhausted all of his dialog cutscenes.

14. Jesus Christ, Denton
After meeting Walton Simons during your next visit to Manderley's office, follow him back to the NSF prisoners, then kill them in front of him and talk to him a few times. He will say this line and earn you the achievement. If you don't kill them, Walton will eventually do them in himself.

15. Reporting for Duty Free
After meeting Walton Simons, drink ten bottles of alcohol or one zyme, then immediately talk to Manderley.

Expert Usage
Kill a camera, turret, or circular red alarm panel with a sniper rifle or a low-tech weapon like a sword. I accomplished this in the Revision beta tests by upgrading the low-tech skill and strength aug to max, then activating it while wielding the NanoSword and attacking one of those security objects.

Rules of Nature
Similar to Expert Usage, but this time you have to use a low-tech weapon to kill a military bot. They're tougher than security equipment, but they're slow, so I recommend staying just behind them while attacking.

Company Man
Kill Lebedev

Brothers in Arms

Before the shootout in Paul's apartment, slip out through the back. This will kill Paul.

Places to Go, Things to Do
Don't kill Manderley on your last trip to UNATCO HQ

No Excuses
Kill Manderley on your last trip to UNATCO HQ before you and he can have a three-way holocall with Simons.

Brutal Argument
Use a killphrase to eliminate Gunther or Anna. Anna is the easiest; just hack her and Manderley's email computers for the two halves of the password before confronting her.

Company First
I think you have to kill or knock out Lebedev at the 747, then in Hell's Kitchen when Paul's sick find the Datacube he mentioned, but don't send a signal to the NSF, and go back and talk to him. You'll tell him you're not interested in helping terrorists, and if I'm right you'll also get the achievement. This was all that's left of the unfinished "stay in UNATCO" split-plotline.

Precaution
Warn Smuggler that his place is gonna get raided before you leave Hell's Kitchen

Wiretapper
At the NSF airfield, there's a phone in one of the closed rooms. Right-Click it to hear a conversation between Juan and Tong that was unused in the final game, but restored by both Shifter and Revision.

Unconfirmed Achievements
These are all the achievement methods that were suggested by commenters but that I've yet to confirm in-game:

Nonbeliever
Kill an NSF terrorist in the 747 terminal after talking to Paul, then talk to him again.

Heh Heh
Kill all of the gangsters except their leader in the subway near the mole people hideout, then tell him to give you a LAM while you have a full inventory.

Get the Hell Out of Here, Denton
Listen to Simons' "interrogation" of the NSF prisoners until he snaps, then talk to him.

Human Revolution
Jump into and get killed by the spinning fan in the vent area just before the Wall Cloud ship.

My Biggest Fan
Destroy that fan.

Not Quite Convinced
Knock out Juan Lebedev
Koko's Achievements
These are achievements that I haven't yet obtained, but Koko did, and how:

Killing Machine
-Anna Navarre would be proud.
Kill Tiffany at the abandoned gas station instead of rescue her after dealing with the bad guys.
Desperate But Pushing
-Reunite with an old acquaintance.
Same location as above. Just chit-chat with Sandra Renton in the lower middle part of the map. Condition for that is:
Guardian Angel
-Keep Sandra Renton out of danger.
Kill JoJo before he kills the 'tons.
Sniping's a Good Job
-Death to all robots!
Destroy a Page Bravo-3 Peacebringer Bot under exclusively usage of your sniper rifle.
Recommend place for that is the rooftop of the Vandenberg Command Center.

Lockdown
-Shut down every UC Page controls.
Area 51 - Sector 4 / Page's Habitat. Close all 3 blastdoors to the UC by using the control switches next to it.
Highly recommend: Multitools, "a lot of it!" also use your skills.
Fully Prepared
-Enter Page's bunker in tip-top shape.
Drop down into the bunker. Fill your health to 100% as well as your bioenergy before you enter it.
Underground Secrets
-Read the secret to get into a hidden community
Find the book of Eli Curly in Battery Park. It's down the hatch from where you can enter the subway station, lie around on one of the generators. 2nd visit, as I remember short before you enter the Mole Tunnels.
No Unnecessary Risks
-You courteously spared the NSF.
Mole Tunnels: NO interaction with the NSF, not even with it's leader!
Be a ghost, grab the key and leave the map trough the toilet.
Ocean Prince's Achievements
Achievements I didn't get, but Ocean Prince did:

Soda Thief is done by having your Computer/Hacking Skill at Master and then using a Soda Machine. It will give you it for free instead of costing 2 credits.
BeeDoodle's Achievements
These are achievements that I haven't yet obtained, but BeeDoodle did, and how:

A Gentleman's Agreement (Knock out Gunther Hermann successfully) with a very weird specific process:
1.) Trigger Gunther by walking into his line of sight.

2.) Just before he triggers the cutscene, spray with the Pepper Gun (in this state, he will run towards you until he meets your position. Lots of jank from this, like getting him to run into the gold vault, but it isn't notable here).

3.) Hack the computer/use the login to establish the system uplink (Walton Simons will appear in the hologram, but will not talk to you).

4.) Now, use LAMs to slowly eat at Gunther's health (This is mostly trial/error. I used 3 LAMs placed increasingly farther away).

5.) Use tranquilizer darts from the mini-crossbow and run away until he goes down (You are able to completely outrun him after shooting him, he even goes back to green after a little while. I was lucky enough to only need one dart).

Goliath (Destroy a Military Bot with PS20) achieved!
First is location. Personally, I did it in the MJ-12 Facility because there are 2 easy to reach military bots at the armory (which you'd be headed to anyway to recover your gear unless you're trying to get 'A Fresh Start').
Here are the steps after reaching the armory:
1.) Use the code to unlock the armory door
2.) Kill/Knockout the MJ12 troop at the desk and grab your stuff (This will just make things easier in terms of damage)
3.) Use the code you receive on the datavault and get upstairs
4.) Use the login on the Security Terminal
5.) Disable both military bots using the Special Options (They won't actually be disabled, this is more like turning their AI off).
6.) Use your weapons to damage it as much as possible. Recommendations include:
  • Use one GEP rocket
  • Use items (Flares, Lockpicks, etc.) to mark distance, and try and use one LAM as close as possible
  • Save if something doesn't break it (separate, but you can overwrite it's your funeral)
  • Keep damaging it until one knife/crowbar swing breaks it.
7.) After damaging it enough, use the plasma rifle from the upstairs part of the armory. I just used a Multitool. Save, and shoot until it disables+destroys the Military Bot.
8.) Reload that save, and shoot it one less than that
9.) Grab the PS20, and shoot the Military Bot. It should disable/destroy it. (If you need a PS20, I think there's one near the beginning of the section in the detention center.


Rules Of Nature: Kill a Military Bot with a Low-Tech Weapon:
Setup is a lot simpler (for reference, I did this one in the chapter 5 armory section).

0.) Not a 'requirement', but having upgraded Low-Tech does make this take less time (I had enough points from saving up to get to Advanced when I did this, and it took less than a minute to kill them).
1.) Deactivate 1 of the security bots (doesn't really matter which)
2.) Walk up behind the activated security bot
3.) Wail on said bot while strafing left and right to remain out of range of sight/weapons
4.) Repeat 3 until dead. You should get the achievement!

BeeDoodle, I added your Rules Of Nature method to the end of the guide. I had contemplated the achievement in the past, but I figured it was something you needed Expert-Level Low-Tech and a Dragon's Tooth sword to achieve.

[*] Use items (Flares, Lockpicks, etc.) to mark distance, and try and use one LAM as close as possible

7.) After damaging it enough, use the plasma rifle from the upstairs part of the armory. I just used a Multitool. Save, and shoot until it disables+destroys the Military Bot.
8.) Reload that save, and shoot it one less than that
9.) Grab the PS20, and shoot the Military Bot. It should disable/destroy it. (If you need a PS20, I think there's one near the beginning of the section in the detention center.

Goliath (Destroy a Military Bot with PS20) achieved!
First is location. Personally, I did it in the MJ-12 Facility because there are 2 easy to reach military bots at the armory (which you'd be headed to anyway to recover your gear unless you're trying to get 'A Fresh Start').
Here are the steps after reaching the armory:
1.) Use the code to unlock the armory door 2971 to get into the armory, 8138 to open the doors protected by the military bots. I remember finding them in the Bot Repair section
2.) Kill/Knockout the MJ12 troop at the desk and grab your stuff (This will just make things easier in terms of damage)
3.) Use the code you receive on the datavault 3512. It's on the desk. and get upstairs
4.) Use the login on the Security Terminal |MJ12|INVADER| I forget where I found this one.
5.) Disable both military bots using the Special Options (They won't actually be disabled, this is more like turning their AI off).

I got A Gentleman's Agreement (Knock out Gunther Hermann successfully) with a very weird specific process:
1.) Trigger Gunther by walking into his line of sight.

2.) Just before he triggers the cutscene, spray with the Pepper Gun (in this state, he will run towards you until he meets your position. Lots of jank from this, like getting him to run into the gold vault, but it isn't notable here).

3.) Hack the computer/use the login to establish the system uplink (Walton Simons will appear in the hologram, but will not talk to you).

4.) Now, use LAMs to slowly eat at Gunther's health (This is mostly trial/error. I used 3 LAMs placed increasingly farther away).

5.) Use tranquilizer darts from the mini-crossbow and run away until he goes down (You are able to completely outrun him after shooting him, he even goes back to green after a little while. I was lucky enough to only need one dart).
