Card

Inside the rightmost locker.
Use the luminol spray on the lockers to find out the combination.
Combination: 275

Pantry

Doll Manual

Inside the right side of the fridge.
To find out the combination, use the iodine slution on the potatons in the cardbox to reveal letters.
Those letters are directions and the writing above them tells you if you have to read them from right to left or left to right.
Combination: Right Left Left Down Up Down

Rec Room

Letter Chart

The Letter Chart can be found in the suit of armor standing next to the neon sign.

Rules of Chess

The Rules of Chess can be found on the small table, next to the chess board.

Number Note

Use the wooden hammer to hit the letter "C" on the side of the chess board to get a wooden cylinder.
Examine the cylinder in the inventory to get to a small puzzle.
The numbers on the chess board can be used to figure out what to enter.
Combination: Rook Bishop King

Card Calender

Use the pieces you got from the slot machine on the heart shape to start a new puzzle.
Solution:
Afterwards pick up the Card Calender.

Control

Card 1

The first card is behind the intercom.

Card 2

The second card is just lying around below the panel with wires on it.

Card 3

For the third card, you have to solve the riddle with the five small mirrors.
The answer can be found by checking the security footage and zooming out.
Solution:
Afterwards take the card out of the drawer that just opened.

Card 4

To get the fourth card you have to light up the correct of the twelve lights.
The solution to this puzzle can be found by checking the security monitor after the lights went out in one of the rooms.
Solution:
Afterwards take the card out of the drawer that opened ext to the lights.

Centrifuge Manual

This file can be found on top of a box on the shelf next to the sink.

File on Fanatic Bio R

Take the knife out of the sink and use it to slash open the pig pillow.

Research Notes

The notes can be found next to the microscope.

Pod Room

Note

The Note can be fround on the left side on the floor.

Zero Doll

To get to the Zero Doll you have to manipulate the floor by hitting the red circles with the hammer.
You goal is to shift the floors so you can reach the two puzzles on the bottom by the golden door.

Study

Birdman Card

Get it by using the human key from the desk drawer on the human safe.

Sun Card

Open the moon safe upstairs to get the card.
The combination can be found on the moon card you found earlier.
Combination: NIL

Moon Card and Note 1

Get it from the star safe by using the key you got from the human safe.

Note 2

Open the cylinder with the bird on it to get the Note.
To open it, use the bird card along with the knowledge you got from the Polovtsian Secrets.
Combination: MIND

Note 3

Get the third note by using the grenade launcher on the cracked glass.

Polovtsian Secret 1

Go upstairs and find the file on your left.

Polovtsian Secret 2

Use the orange chair lift after having activated it by using the lever in the sun safe and enter the combination.
The combination can be found on a note in this room.
Combination:

Blueprint

Use the orange chair again, but enter a different combination.
The combination is on one of the notes, but has to be decoded by using the knowledge from the Polovtsian Secrets.

D-Team

Healing Room

Note

just pick up the note from the white cabinet.

Trash Disposal Room

Note

Open the locker on the left to get the goggles and toolbox.

You have to open the toolbox with Phi, to get to the Note.
First, open the Shutters with the sliders on the control board.
The combination can be found by looking around the room with the goggles.
Combination: TIC

Then unlock the chute to the left of the control board.
Again, the clue for the combination can be found by wearing the goggles and using some items on the scale with Phi.
Combination: It's the weight of a screwdriver and two screws: 155
Then use the toolbox on the chute.



Afterwards, on Phi's side, use the screwdriver and unlock the toolbox by solving another puzzle.
Solution:

Manufacturing

Blueprint 1, Blueprint 2, Blueprint 3 and Note 1

Can be found by turning left and picking it up from the table.

Note 2 and Note 3

Turn right to find it on the floor.

Alex's Note

Alex, the leftmost robot will give it to you after you disarmed the bomb for the first time.

Locker Room

Note 1

The first Note is in the leftmost locker.

Note 2

To find the second Note you have to twitch on the light for the showers by using the red box on the wall and then check the shower next to the locked one.

Note 3

The third Note can be found after turning on the light for the whole room and conecting the music box in the rightmost locker to the shower by using the hose.

Upd: replayed Study randomnly because it was the room with the most files. Picked up everything again, and ta-da!

This achievement is driving me mad. I've collected all the files from the first run, then checked in with this quide because the game is still showing one not collected file. Got so paranoid that counted how many files I'm supposed to collect in each room and compared to what I have. I got everything. Now I have no idea which fragment I have to replay to pick the note I'm "missing" because man, I've got them all! Arghh

Heads up: I was stuck on this achievement despite acquiring all 36 Quest Files.

Replayed a fragment and picked up a note again and it popped straight-away.
