4) I think this is the key point, I ran from him all the way up to the front of the cave so that I was very, very near the entrance but not leaving the cave. I then killed him there and skinned him to get the Savior's Hide. 5) I escaped out of the dialogue as fast as I could (not sure if this was necessary) then I turned and sprinted to the exit of the cave. 6) Upon exiting the cave, the game acted as if I hadn't killed him and Hircine gave me the ring. 7) I verified that both were actually in my inventory. 8) I made another save and closed the game, reenabled USSEP and then reloaded the game and the save and again confirmed they were both there.
What I did: 1) I followed the quest until I met Sinding in the cave. I agreed to help him kill the hunters. After killing them I made a hard save. 2) Then I closed the game and disabled the "Unofficial Skyrim Special Edition Patch". I have no idea if this was necessary, but I read somewhere that the ability to get both was patched by USSEP, so I did disable it. 3) I opened the game and reloaded that save with USSEP off. I then talked to Sinding as the game told me to. I then attacked him to make him mad.
