This is simply all perks list, I only did this guide because in the internet there is no updated and easy to read perk list. Hope this will be useful for anybody.
Also please tell me in comments if something is missing or wrong.

Magical Hunter

+10% spellpower
50 Body and Willpower
You've learned that arcane strength can be used to augment melee attacks and vice-versa, physical prowess can be translated into magical power. This synergy has made you better at both forms of combat.
Bull's-eye

50 Dexterity and Willpower
Your aim improves with both firearms and magical spells, which results in better placed bulletholes and schorchmarks on your foes.

Inserting enough essences into equipment
Practice makes you the master of the laboratory, they say. The hazardous materials you create take up much less space now and you are also more likely to convert fragile things into gold.

Your habitual use of magical potions has resulted in certain physiological changes. You benefit more from potions and the artificial additives in them have made your body resistant to toxic substances.

Looting through the pockets of the fallen foes might feel a bit low, but you can get lucky sometimes not to mention the resistance to poison that comes with this habit after a while.

Pyrotechnician

Apply 3 points in Phlogiston Torch
Your expertise with the fire-belching tools makes you the bringer of fiery doom! Well, maybe something less dramatic but fiery nonetheless.
Elyctrician
+10% to lightning damage
apply 3 poins in Fulgurous Discarge or Fulgurous Trap

Poisonous Cloudburst

The magic is so strong with you that you are quite literally unstoppable.

33% chance to regain all mana when below 10%
40 Willpower
You have an immense (if erratic) deeper source of magic. Each time you get below 10% of your maximum mana , you have a chance to get fully restored.
Thunderous
+10% to lightning damage
Apply 5 points into Energy Bolt
Your expertise with the Energy Bolt spell grants you greater insight into magical lightning.
Inflammable
+10% to Fire damage
Apply 5 points into Firewall
Your expertise with the Flamewall spell grants you greater insight into magical flames.
Chilling

Apply 5 points into Ice Shards.

Wight

Lady Katarina reach level 10
Wights are even more powerful then Wraiths: in some cultures they are even entitled to lead huge ghost armies and ride on undead flying beasts (note that Lady Katarina is not from that culture, unfortunately).
Scholarly Ghost
+6 Skill Points for Lady Katarina
Katarina wearing all rare (yellow) items
Some magical items resonate each other when they are worn together. A well versed ghost like Katarina can learn a lot from examining the subtle interactions between such items.

Mentor perk is described incorrectly in all other guides as '3 powerups in 2 skills in 1 tree (minimum of a tier one and tier two skill).'
Your guide has the correct description: 'Learn 8 powerups.'

Master gameplay perk is wrong, it's "Max all 3 skills in 2 trees. Ex. Strike 10, Bash 8, Flowing Combo 5, Parry Expertise 10, Fury 8, Bodybuilding 5"

Correction for Thunerous and Chilling (I assume all of the 5 rank based perks, but these 2 I have confirmed) - after reaching rank 5, you must ALSO buy the the mastery that unlocks at rank 5

Finally someone did a list. You're missing on Chilling perk under Thaumaturge.
