Day 1 - Book of Medicine, Symptoms, Medical Card, Restocking
Easy enough for your first day, listen to your patients' symptoms and treat them accordingly following the Book of Medicine
Don't forget to discharge the patients by signing their medical cards



At the end of the day, don't forget to restock your medicines that running low (I forgot to restock mine on first play through).
You do not need to restock everything, only the one that very low (a medicine under 50% or whichever you need)

Day 2 - Physical Symptoms, Dosage Syringe, Medicine Effect, Tool Symptom
Marianne introduces new method of diagnosis, Physical Symptom, Dosage Syringe and fix our Tool Symptom

Tool Symptom
Fully Healed > Almost Healed > Feeling Better > Sick > Feeling Worse > Almost Dead > Dead
The better the health status from the original health status, more money you will earn

When the tool shows;
Almost Dead = You need +5 to completely heal
Feeling Worse = You need +4 to completely heal
Sick = You need +3 to completely heal
Feeling Better = You need +2 to completely heal
Almost Healed = You need +1 to completely heal

The medicine effect can be referred on medical card of the patient
Sometimes less dose gives better effect and sometimes it requires higher dose for the best effect. Physical symptom indicates what the best dosage should be given to the patients. Not everyone has physical symptoms. For some patients who don't show any physical symptoms, try treating them with low dosage first to conserve your stock.

Some patients give hints about how much dosage they need.
Small dosage: 1-5
High dosage: 6-9
Day 3 - Laudanum, Anomalies, Totem, Allergies and Restrictions
in Day 3, the witch introduces Laudanum and the Totem. Laudanum is a potion you drink every time you see an anomaly to retain your sanity. When new patient steps into the clinic, observe your surrounding to find anomaly. When you see an anomaly, drink Laudanum. Don't forget to stock up your Laudanum at the end of the day

Drink Laudanum when you see this anomalies:
Window - (Front window & side window)
>Giant eye peeking outside the window
>A silhouette of Manipulator creature standing outside the window

Clock
>The clock moving counter-clockwise
>On the face of the clock, the eye of a creature appears
> Non-stop pig noise coming out of the clock before it reaches 8pm

Others
>Invisible patient, when patient enter the clinic but you cannot see them
>A scary face near the clock
>The objects floating/levitating
>In patient's medical card, patient's portrait is not their face but a creature
>Cockroaches crawling on the table when look down
>Hearing whispers and the light went off suddenly

Anomalies that will later showup after Day 3
>Using stethoscope: Hearing weird noises from any organs (example: frog sound)
>Using magnifier Something weird with the eyes or mouth (example: more than 1 pupil in an eye socket)

Totem has 4 phases. After each misses, the Totem will tell you what anomaly you missed and change to the next phase

1st phase (A bearded man): Nothing happened yet
2nd phase (Fly face): You missed one anomaly
3rd phase (Fly face with red eyes): You missed 2 anomalies
4th phase (Cockroaches crawl out of the Totem): You missed 3 or more anomalies






The Totem will warn you when new day started if there would be anomalies. If the Totem didn't warn you before hand, the day is free from anomalies.

The more anomaly you miss, more sanity you lose


The last patient of the day introduces allergies restrictions.
Check patient's allergens. Avoid giving the certain amount of dose if the patient has allergy of listed things. Giving wrong dosage to patients with allergies, will result in negative effects

Day 4 - Alternative Medicine
Day 4 introduces Alternative Medicine method of diagnosis. When patient comes in the clinic with no symptom, test the alternative medicine on the patients and observe their response to them.

Test either Vital or Digestive alternative medicine.
If positive for Vital: The med must be between Blood or Phlegm
If negative for Vital: The med must be between Yellow Bile or Black Bile
Vise versa for Digestive Alternative Medicine
Low and High alternative medicine work the same way
Day 5 - Humor Circle, Humor Device
Here come the tricky part. In this game, you can treat symptom sometimes with different medicine according to Humor Circle. In this moment, you need to try giving the patient medicine and observe the effect on them. Use that effect to find the right medicine for their illness using Humor Circle.

EXAMPLE

Tool Symptom: Almost Dead
Patient's Symptom: Blurred Vision

Allergy: None
Medicine Effect: 7 doses Black Bile = -2 => put this effect in humor circle

Keep in mine the symptom mentioned by the patient = Blurred Vision thus indicate it to be needing Blood Medicine

There are two '-2' effects in the wheel. Scroll 7 dose of Black Bile towards any of '-2' effect

In this situation, I choose to scroll it to '-2' at the bottom left
Let's test 6 doses of Blood Medicine, it should give us +3

The effect of 6 doses of Blood medicine = +2 => move the wheel slightly so 6 doses of blood is at '+2'

The position of the wheel matched:
7 doses of Black Bile = -2
6 doses of Blood = +2



Let's try 5 doses of Blood Medicine, it should give us +3
Adjust your wheel based on new information gathered
Let's try 8 doses of Blood Medicine for +3
Finally got it!










At this point of the game:
>Try and error if patients have no symptom is important
>Cooperate Alternative Medicine to narrow down the effective medicine
>Always check the Medicine Effect
>Avoid triggering patients' allergies with wrong dosage
>Conserve your medicine stock by testing with low dosage of medicine
>Do not need to achieve perfect Medicine effect. You can keep using +1/+2 medicine as long as it healed the patient and decrease the use of the Medicine
>Treatment slots. Patient has maximum number of medicines they can receive in a visit. Be mindful of this slot
Day 6-10 - Priest's Quest, Magnifier
Day 6

Day 7
The priest comes to our clinic to give us a list for Priest's Quest. Refer the list when the patient from the list enters the clinic for the first time.
This quest need to be done to get achievements: Ending: A new cycle begins & Ending: Fleeing evil, becoming prey

Day 8
A hooded man gives mysterious purple potion which needed to be used on female patients. Fail to abide by the order, the owl will be yoinked
At the end of Day 8,an option to buy Magnifier (use for diagnosis)

Day 9
Marianne brings magnifier ordered on Day 8
Next patient after her requires us to use magnifier to find a symptom and its medicine

Look out for anomalies that may appear while using magnifier. Check EVERY patient on the day that anomalies will appear

Day 10
Captain Thomas comes to the clinic. At this point of the game there is no medicine that can heal him YET

Day 11-15 - Grave keeper's Quest, Fishman Symptoms & Treatments
Day 11

Day 12
Nigel the grave keeper offers a reward for every people we killed in his list. Use wrong medicine or patients' allergies against them
This quest need to be done to get achievements: Breaking Bad & Merchant of Death

Day 13
*Nothing new

Day 14
Level 1 Fishman enters the clinic but no treatment page yet (I'm not sure if every other people receive Fishman patient on Day 14)

Day 15
The librarian gives treatment pages for Fishman symptoms. Patients inflicted with this illness show physical symptom.

Notes:
> Symptom on face or body - Use magnifier or stethoscope to check symptom on patients' body
> The physical symptoms indicates amount of should be used dose and if its vital/digestive problem
> Listen to patient's statement on their symptom. The statement is important to give the exact medicine (Blood/Phlegm/Yellow Bile/Black Bile) . Fishman illness is slightly different because you need to diagnose and treat them based on Fishman page
> The effect of medicine is divided by 2/3/4 for patients suffering with Fishman symptom. To find it real value, multiple the effect with 2/3/4. The real effect of medicine on Fishman is when given the mixture of 2/3/4 or more medicines

Level 1 - Multiple the 1 medicine effect with 2
Level 2 - Multiple the 1 medicine effect with 3
Level 4 - Multiple the 1 medicine effect with 4

EXAMPLE
Physical symptom: 2 claw marks
Patient's statement: Hearing whispers from the sea

>According to the Transform Level 1 Fishman treatment guide, the 2 claw marks physical symptom requires 1-3 doses of medicine
>Patient was hearing whispers = Blood Medicine

Other symptom is: The eye using magnifier (Not every Fishman has this symptom)
>This symptom indicates the patient has Vital problem

Let's test the 2 doses of Blood Medicine on the patient
Since this condition makes effect of medicine divided in half, unless more than 1 medicine mixed together. We can conclude real effect of 2 doses of Blood Medicine equal to +2

Put the REAL value on Humor Circle

Let's try 3 doses of blood + 6 doses of phlegm
(+3) + 0 = +3

Mixing 2 medicines gives REAL effect to Fishman patient

Refer to Level 2/Level 3 Fishman treatment if Level 2/Level 3 symptoms show up on patients
Level 2 - Medicine effect divided by 3 > Real effect need to mix 3 medicines or more
Level 3 - Medicine effect divided by 4 > Real effect needs to mix 4 medicines or more

TIPS
You can use same medicine but need to be separated by different medicine in between. For example = 2 phlegm + 2 blood + 2 phlegm + 2 blood = 4 phlegm + 4 blood

From what I've seen in my own playthroughs doing the Grave Keeper quests does *not* seem to affect which ending path you get.

- I just kept healing him, I realized he was the cannibal the 2nd time he came around. At day 23 he killed me and the achieve unlocked.

To get the "Bankrupt" achievement, you need to spend all your medicine supplies and have 0 coins with you.

@Cutiefangs: It takes a couple of days at 0. At some point the sherrif will come in and inform you that due to financial issues he can´t allow you to keep the clinic open. I am unsure if its a fixed numer of days the game gives you to get out of the red or if the game "mercy kills" your run once it detects you can´t restock but if you keep just letting days run by without making cash it will pop

Hey glad to help :) @Emmy: While that´s not how it´s described in the trophy list, "Oops!" requires you to kill a Fishman specifically, so you got that right. It´s important that you kill them with the first injection you give, it´s not about actual speed it has to be the first injection on that patient. If that is what you did you could hop onto the games discord and report it as a bug, the devs appreciate the help :)

Help! I cant get the Oops! achievement. I've killed regular pts, lvl 1, 2, and 3, fishmen with quick injections. I can't seem to get it to pop??

Could be wrong, but I'm pretty sure the "My Honey Pie" ending is just heal Clark Riceby with the Hunger Medicine (Black Bile) each time he appears and that gets you that. I could be wrong about the medicine since I wasn't paying close attention but essentially just do what you think will heal him. I got it on day 23 entirely on accident because I was just trying to be a good doctor lol.
