Will Artyom Die?
Just like the previous two games in this series, Metro Exodus also has two drastically different endings. At the end of the game, Artyom gets gravely injured and he needs a blood transfusion - in the good ending, he survives and becomes the new leader of the Order. He and Anna start creating a new society.

In the bad ending, Artyom dies. In the afterlife, Artyom meets Eugene, Bourbon, Khan, and Miller. The main hero boards the train and starts his never-ending journey. In the meantime, Anna buries her beloved.
How Do The Morality System And Choices Work?
Let's start with the fact that every large location (the Volga, the Caspian Sea and Taiga) has its own morality system. There, you will have to make moral choices that are usually about killing/stunning enemies. Depending on the decision you make, you will have a different ending for a given chapter - stunning enemies and completing side quests will give you a good ending where e.g. one of your comrades survives.

Also, the game's ending depends on how many companions have survived until the end. Some of the dialogs or cut-scenes are also affected by e.g. death of a specific character. Your friends can also celebrate the fact that everyone has survived.

Some of the enemies can surrender during a fight - killing them leads to the "bad" ending, stunning them will get you closer to the "good" ending.

In conclusion: Moral choices affect your allies, leading to different endings.
How To Get Good Or Bad Ending?
How to get the good ending in Metro Exodus?
To get the good ending in Metro Exodus, at least two of your friends must survive until the end. The characters that can die during the game are Duke, Damir, and Alyosha. At the end of the game, Artyom will need a blood transfusion - this requires more people to be present during that moment. If you persuaded/helped your allies - Artyom will live. He will become the new leader of the Order, and you will get the good ending.

How to get the bad ending in Metro Exodus?
The bad ending is only possible if you don't have enough people for the blood transfusion. In the afterlife, Artyom will meet a few familiar people from the past (Bourbon, Khan). After that, you will see a cut-scene from Artyom's funeral.
How To Make Everyone Survive Until The End?
Your choice can lead to deaths in your company. However, there is a way to keep Duke, Alyosha, and Demir alive. These choices will result in the good ending. Check our guide if you plan on keeping everyone alive. The most important rule - if you have a chance to stun human enemies, do it. The same goes for side quests (focused mainly on rescuing others). If possible, avoid fights with humans. The chapters listed above have all the details on how to keep your friends alive.

Here is a short summary:

Duke - Don't kill any of the cult members in the Volga. Choose stealth approach when you are escaping from the church, steal the boat, and enter the cult leader's base.

Damir - Help slaves in the Caspian Sea (rescue them, complete side missions). Stun enemies when you and Damir are looking for water (don't kill any of the slaves). You also have to rescue slaves kept in cages. Another step is to get the photo for Giul in the bunker.

Alyosha - In Taiga, you will encounter e.g. enemies using crossbows - don't kill them. You need to focus on stealth throughout almost the entire chapter. Thanks to that, Alyosha won't get injured.
How To Save Duke's Life?
There are 3 important choices in the Volga that will determine the chapter's ending. To help Duke survive you have to:

Escape from the Church unnoticed. After swimming into the Church, reach the upper floor. Escape through the window when the guards start knocking on the door. You now have to sneak and stun enemies. Reach the boat and swim away.

Steal the boat in the boat without killing the traders. Sneak and stun the traders. Steal the boat without killing anyone.

Reach the cult leader without killing his men. Inside the cult leader's headquarters, focus on sneaking. Reach the leader unnoticed. Don't kill anyone with your knife - this will have a negative impact on the ending.
How To Save Damir's Life?
In the Caspian Sea, you will encounter slaves' faction - they are forced to work for the local Baron. These people are innocent, driven by fear. The key to making Damir join us is to rescue the slaves, sneak past them and stun. At the end of the chapter, you will have a few side missions where you have to rescue slaves.

The photo of Giul's mother is in the bunker you visit during your searches for a satellite map. Give Giul that photo. Thanks to these steps, you will get the good ending at the end of the chapter. Damir will join Artyom's company.
How To Save Alyosha's Life?
Alyosha's fate is connected with our attitude towards the locals. Avoid killing the enemies - spend the majority of the chapter on sneaking and stunning human enemies. Going through settlements during the night is a good idea. Luckily for you, the majority of the enemies are carrying a crossbow - you will have plenty of time to run towards the opponents and stun them. If you come across prisoners, always rescue them. Thanks to that, Olga will appear at the end of the chapter - he will let you and Alyosha go in peace. Otherwise, Alyosha will end up in a wheelchair.
How To Find The Real Baron In The Caspian Sea?
One of your goals here is to eliminate the Baron. However, there is one small problem - this location has two Barons! One of them is the real Baron, the other is an impostor.

I Marked a Location with a Green Star in The Map, in my last guide (The Caspian Sea Upgrades). Speak with the man you found in that location. He will tell you his story and inform you that the current Baron is an impostor.

The Baron will tell you about his plans, about the holy flame, and other interesting things. After that, he will wait for you to make a decision. You can kill the man or leave him be. The man won't defend himself.

Does Killing The Real Baron Have An Impact On The Chapter?
Although the Baron's character is crucial for this chapter, killing the real Baron won't affect the plot or the ending of this chapter. Leaving him alone won't change Damir's and Giul's fate, too. Also, killing the real Baron won't change your karma - the game won't count this kill.

The only potential difference is that you can hear a different report from the Caspian Sea (while using the radio in the Aurora). For example, leaving the real Baron alone can be noted in Giul's report.
How To Get The Good Ending In Sam's Story DLC?
To get the good ending you have to:

During your first encounter with the Captain, lower the weapon and stop aiming at him.

Help the Captain move the wooden barricade during the second meeting with him.

Let the sniper who shot at you live - stun the enemy.

When you meet the captain in his apartment, share your food with him as soon as the lights go out.

While you are at his apartment, set 5 traps with Molotov cocktails before heading out.

Search the area after defeating the bat demon. One of the question marks will lead you to the crew's bodies - find them for the Captain's sake.

Once you get the detonator at the end of the game, decide that you want to blow up the boat.
