How to obtain the Maria Ending

The Maria Ending is the hardest of the three normal endings to obtain. Most of the actions are heavily reliant on the point system that influence the Maria Ending. It’s best to actively try for this ending on its own.

Woodside Apartments: In the Coin Box Puzzle, select the Woman Coin for the final solution.

When Maria first joins you in South Vale, have as many optional conversations with her as possible – such as by trying to enter the Silent Hill Historical Society.

Pay Attention when she says you are going the wrong way, stick to the main path.

Don’t break glass when she is with you if possible. You will have to break the window of room 108 to open door to let her enter the room.

Do not let Maria take too much damage when she is with you.

Take Maria with you to explore Nathan Avenue before you go through Jacks Inn. This allows for a number of additional interactions, and reportedly lowers the points you've earned for "Leave" and "in Water." Just make sure she never takes a hit.

Don’t be too far away from her for more than 5 minutes.

Don’t listen to any of the cassette recording.

Do not inspect the Photo of Mary, Mary’s Letter or Angela’s Knife in your inventory.

James should avoid examining personal items in general. For the Coin Box Puzzle, the Woman Coin should be used as the final answer.

Talk to her when she’s holding the dress upstairs in Jack’s Inn, and as soon as possible when she’s standing in front of Baldwin Mansion.

Stay very close to Maria for at least 10 minutes.

Inside Heaven’s Night, interact with the pole, posters, costume, and "Lost & Found" box before advancing. (A good place to stay close to her for 10 minutes)

Don’t look at all of Laura‘s drawings (rumored – not verified)

When in Brookhaven Hospital and the Labyrinth, check in on Maria regularly as you progress.

When Maria stops to rest in room C5 of the hospital, talk to her 3 times in a row until she turns around. (you can get two cutscenes with Maria in the Hospital, one after collecting the first bracelet and another after collecting the Rooftop Key). Return to room C5 one last time after solving the Director’s Office Safe for another cutscene.

In the Otherworld Brookhaven Hospital, place the Lead Ring on the top hand and the Copper Ring on the lower hand.

During the Labyrinth, check on Maria after finishing each cube. (rumored-not verified)

When doing the Mirror Puzzle in Lakeview Hotel, place the Ripe Apple on the plate instead of the Rotten Apple.

When you get the Scarlet Egg and Rust-Colored Egg at the end of the double pyramid-head battle, go through the door that you place the Scarlet Egg into.

Do not listen to the full conversation in the long hallway before the final boss fight. Run through the long hallway as fast as possible.

If you have enough points with Maria she will be lying on the bed instead of standing at the window.
You will get the Maria Ending after the boss battle.
