Foreword / Breakdown
Return to Castle Wolfenstein's original campaign features seven missions (or chapters) spanning 26 maps. RealRtCW v5.0 takes things a step further by adding in a hub map, Malta Headquarters, which the player revisits in between certain missions. Of the current total of 100 achievements RealRtCW has in store, 59 of them are to be unlocked through the thus expanded OG campaign.
This guide is created keeping both the returning players and the newcomers in mind and aims to serve as a companion for any kind of OG run by covering all of the secrets and the achievements. Still, a total of 17 achievements are purposely left out of it to save some of the space needed for the details. For the record, they are the ones tied to completing the missions on lower (7) and higher (7) difficulty levels, as well as the ones related to beating the bosses (3).
Thanks to RealRtCW's extended options menu, it is possible to customize every run down to its smallest detail. Below are three 'builds' I came up with while working on the guide that one may opt to try - think of them as going from 'the most vanilla' to 'the most modded'. Note the lack of 'Additional Enemies', 'AI Respawn' and 'Health Regen' options as I thought of them as 'No' for all three. Mix and match to your liking: the secrets and the achievements remain the same.

M1: Ominous Rumors
This mission consists of 3 levels that contain a total of 11 secrets and 16 treasure items.

ESCAPE! (escape1)
Secrets: 7 / Treasure Items: 9 / Rewards: Armored Helmet
1/7 - Once you reach the spiral stairs, descend all the way to the bottom and destroy the planks with the sign 'Munition' on them to get some extra rounds. Unlocks -
Hidden Treasure Find the secret area.
2/7 - Following on from the previous secret, go up the stairs this time and get through the door with the red bulb hanging above it. In the comms room that follows, destroy the board by the alarm button to find two treasure items.
Tip - Better check the next door on the stairs at this point to take out the guard on the roof.
3/7 - Once more on the stairs, step out to the courtyard via the door you passed on your way up and get into the barracks room found to the corner. Look for a breakable part on the floor amidst the bunk beds to find another treasure item.
4/7 - As soon as you drop down to the lower floor through the hole created by the guards' fire, destroy the arched part of the wall across the door to reach the room on the other side.
5/7 - Following on from the previous secret, go through the door signified by torches in the next hall and interact with the unlit candelebra on the wall to reveal a hidden compartment behind the fireplace within the room that holds three treasure items.
6/7 - Following on from the previous secret, turn left as soon as you step into the next room and destroy the portrait hanging above the table to reveal two more treasure items.
7/7 - In the next hall, interact with the left one of the two tablets mounted on the wall to access the hidden room behind that houses the final treasure item of the level.

CASTLE KEEP (escape2)
Secrets: 4 / Treasure Items: 7 / Rewards: Bandages
1/4 - Right at the beginning, go through the door to your right to reach the commander's room. Get rid of him and shoot the flag hanging above the fireplace to reveal a compartment behind it. Interact with the painting on the wall and pull down the lever within to open up the compartment in question. Finally, place one of the chairs in front of the fireplace (as shown) and utilize it to jump and grab the two treasure items above.
Tip - Note the barred room labeled 'Munition' here, as it becomes accessible after the next step.
EX - Once you reach the courtyard, go through the lower door with the red bulb hanging above it first. You end up on the upper floor of the starting area, also unlocking a shortcut on the way via interacting with the door in the connecting corridor.
Pull up the lever labeled 'Munition' to the back of the comms room, return to the commander's via the shortcut and check the now unlocked munitions storage to get ahold of the unique 1905 revolver, as well as some ammo for it and various supplies. Unlocks -
This Belongs to You Sir Retrieve Agent's One personal weapon.
2/4 - After crossing the courtyard and taking the following stairs down, get into the wine cellar first and look for a slightly jutting piece of stone on the pillar between the barrels closer to the door. Interact with it to reveal a compartment to the back. Use 1938 Latour to unlock -
Valuable Property of the Reich Do some wine tasting.
3/4 - Taking the next set of stairs down, get into the room where the alarmed officer is panicking. Destroy the right one of the two adjacent boards and the wall behind it to get two treasure items.
4/4 - Following on from the previous secret, first take the door in the room you are in and then the one directly across with the red bulb hanging above it. Shoot the sole barrel found inside to reach an otherwise locked room that houses assorted goodies.

TRAM RIDE (tram)
This level has no secrets or treasure items.
It's Just a Scratch! Apply the first aid.
* If secret rewards are enabled, use the bandages you are awarded for getting them all in Castle Keep. Otherwise, the first one of its kind is to be found within Karl's place in the next level.
M2: Dark Secret

VILLAGE / WULFBURG (village1)
Secrets: 2 / Treasure Items: 3 / Rewards: Armored Helmet
1/2 - Right after meeting Karl, the following stairs takes you to an overpass. Once you crawl your way under it, go through the door on the right and take a left. Look for a slightly jutting book in the bookcase within the room you end up and interact with it to reveal a hidden floor beneath.
EX - Following on from the previous secret, take to the right at the fork on your path and climb the pipe found by the next corner. Traverse the ledge, loot both of the nests for extra health and ammo and drop back down to the fork in question.
2/2 - Following on, hop on the railing in the next area and make your way to the balcony at the corner where health packs are laying out in the open. Do not forget to drop by the storage room below before moving on.

CATACOMBS (crypt1)
Secrets: 1 / Treasure Items: 1 / Rewards: Grenades
Knowledge Is Power Read the book.
* Once in your possession, use Dr. Zemph's journal as any other inventory item to read it.
1/1 - Right after getting a hold of Dr. Zemph's journal and climbing your way to the upper floor, look for a slightly jutting piece of stone on the dead end to the right of the fork. Interact with it, turn around and sprint to the now accessible alcove on the catwalk.
Tip - Note the ordering of the tablets within, as they act as a hint for the puzzle in the next level.

CRYPT (crypt2)
Secrets: 2 / Treasure Items: 2 / Rewards: Medium Health
1/2 - Once you push the button at the dead end and deal with the ambush it triggers, turn back and shoot the cross standing on the corner to make way to the barred crypt on the lower floor. Be careful not to get crushed by the thing though.
2/2 - Solve the switch puzzle and step onto the pillar that acts as an elevator. Once you pull down the lever on the second floor, you are ambushed by the undead coming out from both sides. Get through either of the openings to discover a treasure item. Unlocks -
Gold Rush Find the chest full of gold.

THE DEFILED CHURCH (church)
Secrets: 1 / Treasure Items: 1 / Rewards: Dynamite
I'm On Offense! Inject yourself with a syringe of adrenaline.
* Get the adrenaline M-1 shot laying on the altar and use it... or save it for the upcoming boss.
Tip - If you are having trouble by the ladder thanks to the grenadier, get on top of the altar, toss one of yours through the window above and listen for the thump.
1/1 - As you traverse the second floor, drop by the ledge overlooking the altar and from there, jump over the railing towards the ledge to reach a side room housing assorted goodies. Unlocks -
Dr. Jones? Find the crystal skull.
Another achievement to better get in this indoor level, rather than in the outdoors one later -
Foot Fetishism Kill elite guard using your foot.
* The bunch guarding the exit provides the best opportunity, as they do not have much space to maneuver around. Weaken the last one with some SMG fire then go in for the kick o' death.

Tip - Be sure to visit the medical bay every time you drop by Malta. A gradually improved shot of adrenaline waits for you to pick it up, which comes in pretty handy against the bosses.
1/1 - Make your way to the south district and look for climbable steps on the watchtower next to the tavern. Go up and get in through the window overlooking the shore to find a treasure item. Instead of climbing back down, go up another floor and jump into the water to unlock -
Airborne Training Do a leap of faith.

FOREST COMPOUND (forest)

1/1 - Once you are in the compound proper, look for a bunch of crates stashed by the inner gate. Take care of the officer dwelling in the smaller building first, then utilize the said crates to get on top of the perimeter wall. From there, make your way to the bigger building's roof and drop inside via the opening. Shoot the fuel canister found within to reveal a solitary treasure item.

ROCKET BASE (rocket)
Secrets: 1 / Treasure Items: 2 / Rewards: Panzerfaust
Optional - A stealth entry is possible as long as some care is taken. Opt for the left side as you exit the truck and stand by as the crane operator ahead talks through the intercom. Get behind the crates next to the crane as the dialogue continues and step in when the said crane takes off. Riding it down lets you bypass the initial area without firing a shot. Note that the train you land on can also be activated from the nearby console for a covered ride through the following tunnel. Descending to the lower floor through this stealth route unlocks -
Infiltrator Use an alternative way at the rocket base.
1/1 - Right after crossing the catwalk that a guard tries to blow up, you reach a command room. First destroy the map on the wall to reveal a locked compartment, then interact with the button found on the underside of the nearby desk to open it up and get the treasure items within.

RADAR INSTALLATION (baseout)
Secrets: 1 / Treasure Items: 2 / Rewards: Flak Jacket
EX - After clearing the area around the MG post, get into the nearby depot and jump on top of the crates within to find a rather rare M1911 pistol alongside some ammo to go with it.
1/1 - As soon as you ascend the outer stairs, go through the right one of the two doors on your path to find yourself in a workshop. Destroy the grating on the floor and crawl through the duct to end up in an otherwise locked munitions storage.
After looting the place and taking the stairs, step outside for a moment and get the two treasure items laying on adjacent desks in the nearby comms room.

KUGELSTADT (sfm)
Secrets: 1 / Treasure Items: 0 / Rewards: Gas Grenades
1/1 - Once the Allied Panzer you escort reaches the facility, it breaches the entrance for you to move in. After clearing the immediate area of the enemies, climb the fallen beam that lies closer to the door and jump across to get the assorted goodies residing on the corner.
Tip - In case the jump is giving you a hard time with realistic movement enabled, try it with the knife at hand instead of a ranged weapon.

THE BOMBED FACTORY (sfm)
Secrets: 2 / Treasure Items: 3 / Rewards: Health & Armored Helmet
Note - This level originally featured no secrets at all.
Tip - Better clear out both the surrounding area and the sewers before going for the first secret.
1/2 - After clearing out the starting area, instead of rushing into another MG fire, drop down to the sewers through the opening amidst the ruins. Once down, take to the right from the fork to get inside the first MG post. Notice the red-lit button on the wall: it opens up the gate by the entrance, yet the said gate remains open only for a limited time (around 75 seconds).
So interact with it, drop back to the sewers, resurface next to the halftrack on the other end and sprint to the gate in question to get ahold of the unique PPSh41 SMG, as well as some ammo for it and two treasure items. Unlocks -
Reverse Lend Lease I Find the PPSh.
2/2 - Shortly after stepping onto the second floor of the rundown building, you overhear two guards talking about disarming a bomb. Once they blow themselves up, drop down to the lower floor and destroy the damaged part of the wall by the gate to discover another treasure item.

THE TRAINYARDS (trainyards)
Secrets: 2 / Treasure Items: 2 / Rewards: Flak Jacket & Panzerfaust
1/2 - Before moving into the warehouse, get on the roof of the nearby building via the ladder on its side. From there, sprint jump towards the railwagon with an opening on top and drop inside to find assorted goodies.
2/2 - On your way to powering up the elevator, get on the railing facing the shutter door and sprint jump onto the platform before it. From there, hop onto the control platform to get into the adjacent room full of supplies. Be sure to pull down the lever on the console for a safer return.

SECRET WEAPONS FACILITY (swf)
Secrets: 4 / Treasure Items: 0 / Rewards: None
1/4 - Right at the beginning, take care of the welcoming guard then shoot the group of canisters on the left to break into the otherwise locked armory.
Damn, You're Ugly! Kill loper using panzerfaust.
* The poor loper in the following secret provides a good opportunity, as it is also stationary and confined to its cell. Equip your panzerfaust before interacting with the button, quickly step back once you do and fire away in its direction as soon as the door pulls up to put it out of its misery.
2/4 - Shortly after descending to the testing area, interact with the red-lit button on the pillar to open up the adjacent chamber where a loper is laying in wait. After taking it down, destroy the cracked part of the back wall to reach the supplies residing on the other side.
3/4 - While in the room with a trio of torpedos just before the sub bay, destroy the grating by the pipes in the water and swim through. Make your way up and follow the piping around the corner to find a ladder. Climb it, then interact with the panel on the other side of the catwalk and turn the valve within. Although this secret offers no supplies by itself, it prepares the way for the next.
4/4 - Following on from the previous secret, directly drop down to the catwalk on the lower floor and dive into the rising water. Swim through the opening within the tunnel around the corner to reach the otherwise locked storage room from before.
M5: Deathshead's Playground
This mission consists of 3+1 levels that contain a total of 2+1 secrets but no treasure items at all.

Once on the other side, drop down to the ledge and utilize it to reach the door in sight. Stepping in, get the miniature residing on the table to unlock -

X-LABS (x-labs)
Secrets: 1 / Treasure Items: 0 / Rewards: Panzerfaust
Superiority of Flesh and Blood Kill the proto soldier using your knife.
* The first proto soldier you ever come across on the second floor of Lab 2 actually provides the best opportunity. Surprise him with a rocket to the back that takes away half of his health, spray him a little with Venom, and finally move in to make the killing blow with your knife.
1/1 - Progression requires you to take a dive upon descending to the first floor of Lab 2. Do so, and once you reach the open area underwater, swim through the gray gate to a fork around the corner. The path straight ahead leads to your second objective. Take the right path instead and go through two more gates to end up in the otherwise locked storage room found in Lab 3.
After that, grab the Project Book from Lab 1 and go for the second objective via another dive.

MALTA HQ: 4TH VISIT (malta_4)

The fourth visit to Malta HQ starts in the training facility, where the various weapons you collect throughout the missions are stored and made available for target practice on demand. Unlocks -

1/1 - Leaving the harbor behind, take a left to reach the museum grounds. Climb the ladder to the right of the gate and make your way along the ledge to the balcony in the middle. Once you get the two treasure items laying here, hop onto the next balcony and look for a slightly indented part on the wall near the sculpture. Destroy the said part to uncover an easter egg and unlock -
Song for Elise Find Wide Franklin.

BRAMBURG DAM (dam)

1/1 - Right after clearing out the maintenance corridor, interact with its button without stepping on the lift itself and destroy the grating underneath to break into the adjacent room from below.

PADERBORN VILLAGE (village2)
Secrets: 5 / Treasure Items: 12 / Rewards: Flamethrower

1/5 - Right after taking out the first officer, drop by the adjacent room and destroy the lid of the bedroom bench to reveal the three treasure items stashed inside.
2/5 - Following on from the previous secret, destroy the padlock on the half-size door midway through the tunnel to break into the alchemist's cellar, where three more treasure items await. Carry on to find the alchemist buried in his craft. Get him before your cover is blown to unlock -
Dr. Strangelove Put an end to his magic.
3/5 - Following on from the previous secret, shoot the sniper who is directly ahead of you as you step out of the alchemist's place, then get into his nest via the pipe by his window. It might be a good idea to take out the guard waiting by the hotel before making that move, though.
4/5 - After listening to Burkhalter whining about his drink, descend the stairs by his window and get into the wine cellar. Take care of the annoyed guard, then interact with the bung of the wine barrel on the far corner. Doing so reveals a hidden passage next to it that holds three treasure items and doubles as an alternate entrance to the general's residence.
5/5 - Assassinate your last target, then destroy the portrait hanging between the bookcases to reveal a lever. Pull it down to open up a compartment within the room where the unique Mosin Nagant rifle resides along three treasure items and lots of ammo. Unlocks -
Reverse Lend Lease II Find the Mosin rifle.
Before leaving the area, go through the door by the stairs that leads to this residence to find out who stole the prized wine the other guard was hopelessly searching for.

CHATEAU SCHUFSTAFFEL (chateau)
Secrets: 2 / Treasure Items: 7 / Rewards: .30 Cal
1/2 - Upon reaching the main hall, turn around and shoot both the portrait hanging above the fireplace and the swastika behind it. Doing so unlocks the door found on the side of the grand staircase. Get in there before it becomes locked again to reach the study underneath.
EX - Before ascending the grand stairs, check the other side of the floor to come upon a coven taking place. Deal with those involved and get the two treasure items residing on the altar.
2/2 - Upon dropping down from the roof onto the second floor, interact with the shovel found by the fireplace to reveal a hidden room where four more treasure items reside.

UNHALLOWED GROUND (dark)
Secrets: 1 / Treasure Items: 9 / Rewards: Bandages
1/1 - A partly destroyed building can be seen to the left as you get by the first corner on your way to the dig site. After turning the second corner and securing the immediate area, take a closer look to find the damaged parts of its floor. Destroy them to access the room below that holds nine treasure items as well as various supplies.
M7: Operation Resurrection
This mission consists of 3+1 levels that contain a total of 4+1 secrets and 9 treasure items.

THE DIG (dig)
Secrets: 2 / Treasure Items: 3 / Rewards: Panzerfaust
1/2 - After taking care of the initial guards, climb the right chord of the bridge you come upon to its topmost point. Turn right, jump over to the ledge and crawl in to discover two treasure items.
Optional - It is possible to get rid of the first proto soldier you encounter in this mission without directly killing him. As soon as you cross the bridge, crouch and stick to the right cavern wall till you reach the crates stashed ahead. Doing so should trigger the proto soldier to move, yet not alert him to your presence.
Wait by the bigger crate for ten seconds, at which point you should hear rocks crumbling. Move out of your cover with panzerfaust in hand to find him standing on the rock bridge. Fire a rocket towards the bridge in question and watch him fall to his doom. Doing so unlocks -

EX - Once you step into the sepulchre the undead warriors spring from, be sure to get the two treasure items residing on the shelves before progressing further into the castle.
1/2 - Following on from the previous step, start by clearing the next room from the undead and get the slightly hidden treasure item by destroying the lid at the corner. After that, destroy the rusted bars by the skeleton found below the stairs ahead and crawl in to discover three more.
2/2 - Once you arrive at the inner balcony overlooking the knight statue, follow the path created by the fallen beam on the right to break into the otherwise locked room on the same floor.

Note - The following jumps are much easier to perform with realistic movement disabled.
1/1 - Drop by the medical bay and get the shots of adrenaline. Take a left from there and head to the south district via the door around the corner. Nearing the church, go through the open door to your right and make way your to the opening above utilizing the crates and the beams within.
Once through, get on the railing and sprint jump onto the scaffolding by the church. Injecting the adrenaline M-4 shot, sprint along the roof and jump towards the balcony located to the back of the church. Go through the door and keep on the path to end up in a curious storage room. Read the note on the table and get the crate of champagne on offer to unlock -
Wolf's Lair If it's not from Champagne, it is not champagne, is it?!

Still in the south district, go to the nearby hotel and get your keys from the reception. Take the stairs to your room and interact with the bed to relive a moment from Castle Wolfenstein. Pull down the lever by the operating table, then move on through usual means to end this nightmare.

MALTA HQ: 2ND VISIT (malta_2) > Additional Achievements: 4
You Are Not Supposed to Be Here! Get bartender annoyed.
Considering you are coming out of the storage room next to the hotel, go to the opposite corner of the church to overhear a woman speaking about closing early. Get into the tavern to meet the bartender, Evie. Keep interacting with and hear her initial lines, then jump over the counter to her side to get a last line from her, this time pretty much annoyed.
On your way out, drop by the shore and pick up the provided throwing knives from the target practice course found to the very left, as these are required for another achievement further on.
The Black Goat of the Woods Speak to him.
Following on from the previous step, go through the the alley to the back of the church with a blue-lit lantern hanging above it to end up in front of the headquarters. Drop by the medical bay to get your next shot of adrenaline. While doing so, take a moment to check the doctor's room and interact with the ominous painting on the wall.
Espionage Warfare Get a warning from OSA.
Shepherd's Fate Find a way to kill a betrayer.
Leaving the medical bay behind, turn right and get through the illuminated door by the waterway to find an agent waiting for you on the other side with the latest news from OSA. Now that you know about the spy in Malta, have a look upwards in the previous corner to find him checking the alley from a balcony. Get on top of the nearby crates and unsheath the knives you brought along. Fix your sights so that it is in line with the sixth support from the left but only a bit above the roof line (as shown), then jump on the spot and throw one to get him.

The Dots and Dashes - What Do They Mean?! Receive secret radio transmission.
Make your way to the hotel by taking the door to south district found across the headquarters. Get into your room and interact with the transmitter in the bookcase to receive a coded message.

Black Market Find smugglers' hideout.
Leaving the museum grounds behind, return to the harbor via the gate to the right. Go through the illuminated door behind the sergeant, get the chair found by the table and place it next to the crates on the other side of the room (as shown). Get over the crates and interact with the slightly indented part of the floor to find out that it is actually a trapdoor. Take the ladder down, mind the booby trap on the way and get the artifact residing on a crate in the following room.

Go up the slope by the armory and take the door around the corner to reach the chapel, where Agent Two is enjoying some alone time. Keep interacting with her to hear all she has to say.

M1: ESCAPE! (escape1) > Get them!
Escape! Complete the first level in less than 90 seconds.
As long as you have the layout of the level memorized, this speedrun should not give you much of a headache. Disable realistic movement if need be, then start the first mission on 'Can I Play Daddy?' difficulty level so that you take the least damage from enemy fire.
Sprint out of your cell to the first guard, knife him down and ascend the first set of stairs. Take out the guard by the spiral stairs with a knife throw, get his MP40 and hurry to the comms room. Stick to the right and hop once to get the extra ammo from the shelves as you make it out to the ramparts. MP40 the next guard and get his Kar98k in return. Put out the spraying guards from above before dropping down to the lower floor and collecting their SMGs.
From there on, it is simply a mad rush to the exit while braving the bullets coming your way. Make two lefts after crossing the hall of knights and then keep going straight. If all goes well, you can check-out around 70-75 seconds' mark, with ample time left for unlocking the achievement.

M3: FOREST COMPOUND (forest) > 'Is someone there?'
You're Pretty Good! Complete the forest mission on the first try.
Note - 'On the first try' means 'without using the load menu during the level'. For retries, either have a save before leaving Malta HQ to return to, or restart the mission from the main menu.
The Snooper loadout (#2) is the obvious choice for this mission, yet it can also be tackled with the Sten one (#1), so long as you do not mind getting a little closer for the kills. The route you take should be in line with what is drawn as 'covert route' in your journal.
Wait for the first guard by the farm house to get near the window and be on your way. Likewise, pause for a while by the next corner for the patrolling guard to look the other way, then carry on towards the tunnel to the right. Mind your footsteps around the building though, otherwise the officer residing in can notice your presence.
Once across, take out the guard in the watchtower near the tunnel entrance without getting too close, then make your way to the side door found within the said tunnel. Stick to the path until you end up in the vantage point overlooking the MG post. Without taking the slope down, wait for the dialogue to conclude and the duo below to go their separate ways. The moment they are apart, you need to Snoop all the three enemies here in quick succession. Go for the sniper first, the now alerted gunner the second and the unaware officer the third.
Now that the trio is out, take position by the watchtower and snipe both the guard patrolling by the gate and the officer in the lookout above. After that, get closer to the gate and wait for the sniper on the water tower to come around your side. Take him out, get in the compound and look to your right as soon as you step in for the guard patrolling by the crates. Snoop him while he is idling, then make your way to the truck sticking close to the perimeter wall. Be sure to not rush this final step as doing so may alert the last guard who controls the access to the rocket base.

M5: PADERBORN VILLAGE (village2) > 'Nah, must be my imagination!'
Ghost of Paderborn Complete the Paderborn mission on the first try.
Note - As above, 'on the first try' means 'without using the load menu during the level'. So save your game before exiting Bramburg Dam and restart from that save if you are retrying.
Both loadouts for this mission come packed with a Sten and a Snooper rifle, so pick the one you like and not worry about spending the Snooper rounds (.30 Cal) while infiltrating the dam 'cause getting through Paderborn Village is mostly a Sten job. As long as you stick to walking instead of running and know where you are going, you will do fine.
Minding your footsteps, take out the first patrol and get into the nearby building. Sten the guards by the fireplace then the major upstairs and carry on. Snoop the sniper overlooking the next alley and the hotel guard around the following corner and step into the hotel in question. Once again, Sten the two guards in the dining room then the general upstairs. Get into the adjacent bedroom, wait by the window for the guard down the alley to come out and snipe him as the door closes.
Leave the hotel and move in silently through the said door to Sten the next general. Get out via the same door, take the tunnel with a sign hanging above it and wait below the balcony for the general's wine speech to conclude. Carry on to the wine cellar and take the hidden passage into the residence. Deal with the general first, then get back to the first floor and Sten both the guard in the balcony and the one patrolling below in quick succession.
Eliminate your last target next door, then wait by the stairs for the guard across to pause in the corner. Once he is down, get into position by the tunnel entrance below and greet the chatting duo with Sten in hand. Finally, take out the solitary guard patrolling by the chateau with another quick spray and get in through the side entrance.

M7: HEINRICH (end) > 'But I sense the presence of another...'
Henry the Fowler Lure the king into the trap.
The alternate way to deal with Heinrich is more or less the easy way out and not that hard to pull off. For starters, step out of the tunnel slowly with your back turned to him and notice the falling beam that is meant to keep you from leaving the arena once you are in. The idea is to utilize it to take him down instead.
So get a glimpse of the gang to alert them to your presence and simply wait inside the tunnel for them to make their way to your door. More power to you if you got the adrenaline M-4 shot while on your last visit to Malta HQ. Use it to avoid getting suffocated by the spirits the king sends your way in the mean time. As soon as he arrives in front of the opening that leads to the arena, sprint out of the tunnel, turn around and watch the dark master himself get crushed under the beam.

I'd Buy That for a Dollar! Complete any chapter in '999' mode.
999 ammo, 999 health, Death Incarnate. Without a doubt, the easiest of the bunch. Although your HP slowly detoriates to 50 and there are no health packs to be found, the amount given at the start is more than enough. Moreover, every weapon comes packed with 999 ammo, so you can even have some mindless blasting with a panzerfaust.
No Pressure Complete any chapter in 'Ironman' mode.
No manual saves, Death Incarnate. Also disables the checkpoints, so you have to one-shot every level. Still, not much of a disadvantage in the missions that does not feature a boss at the end, as long as you have perfected your gunplay.
Average Hitscan Enjoyer Complete any chapter in 'Hardcore' mode.
No health and armor pickups, Death Incarnate. Yet the checkpoints and quicksaves are in. So try to get through the encounters unscatched, yet retry if too much health is lost in the process. The bandages and the wines you may get ahold of along the way can also be used to recover some.
Didn't Even Break a Sweat Complete any chapter in 'Walk in the Park' mode.
No HUD, Realism difficulty. Not as hard as it sounds since the crosshair is still there and saving is allowed in case any encounter goes sour. Be careful not to get shot as Realism is more or less the insta-kill difficulty with only 25 HP to boot (yet it also affects the enemies in a similar way).
The 2000 Yard Stare Complete any chapter in 'Nightmare' mode.
No HUD, no manual saves, no health and armor pickups, Death Incarnate. Well, this one is the real deal as you have to one-shot every level with the starting 50 HP that does not get refreshed upon reaching the next one. Your best bet is to perfect a route through the first mission. Not take any chances, keep a low-profile as long as possible and make good use of the bandages and the wine from the second level whenever you feel in need throughout the last one.

Get Psyched I Complete the hidden Wolfenstein 3D level.
Get Psyched II Complete the secret Wolfenstein 3D level.
Click on 'Credits' from the main menu then go through the pages until the phrase 'Get Psyched!' takes the place of the 'Next' button. Click on that to find yourself in a touched-up recreation of the first floor (E1M01) of id's 1992 classic, Wolfenstein 3D.
Though the visuals are modernized, the layout and the secrets remain more or less the same. If you already know your your around, hump for them in their usual places. If not, know that there is a secret exit besides the regular one that takes you to another recreation of a floor (E1M10). Accessing it requires you to first interact with the 'pushwall' on the corner of the exit hall, then another one to its right when the first one comes to a stop.
Tip - Check these overhead maps (credit for the originals goes to Christopher Long) in case you need help finding your way around and / or locating the secrets in these two levels.

For "Superiority of Flesh and Blood" I had to kill the proto-soldier with only a knife, I wasn't allowed to do any other damage. He did fight with the looper though. I used the proto-soldier that enters the secret room, since he uses rocket. You can move in, stab him a few times, hope he doesn't hit you, retreat, quicksave and repeat until he's dead.

Heinrich; as it is tipped in the description for the sole secret of Catacombs ('Note the ordering of the tablets within, as they act as a hint for the puzzle in the next level'), secret #2 of Crypt (via the ambush) is only reachable if the player presses the three tablets within the trap alcoves in their intended order. That order is shown in the fourth screenshot for the Catacombs secret.
