After much researching, I decided to combine all the info I found on the Room of Rewards, the rewards inside, and other bonuses from extra requirements involved!

What is the Door to Rewards?
The Door to Rewards is a special room on each floor of Castle Oblivion which requires a special card known as a Key to Rewards, which upon opening will lead to a room with a treasure chest (or two if you have fulfilled the correct requirements). Further in we're going to go a lil more in detail on each requirement for the doors and the specifics of the Key to Rewards drop rate.
The Key to Rewards and it's door specifics
The Key to Rewards is a Key card, similar to that of the story cards, you can hold only one at a time, so don't hoard it the whole game trying to get more for later (unless you're aiming for 100%, in which case we'll get into detail in the next section for it).

But there might be the question "I've never seen a Key to Rewards before! How does it drop?" And to that I will say, that the drop rate actually only starts on the 7th floor onward, the chance increasing the higher you get, with the 7th - 10th floor (I believe, hard to gauge 100% with 100 Acre Wood) having a 15% chance, the 11th floor having a 16% chance, 12th being 18%, and finally, the 13th floor being 20%.

Now! Onto the part that might throw a lot of newer people off, the door requirements are actually NOT world specific, but floor specific! The specifics for each floor however have been found and noted, added on to testing by myself for clarity, so each floor's Door to Rewards requirements is as such (For note, I will not be noting that you need a key for each floor, just expect to need one):

1F (Traverse Town): Only the Key to Rewards is needed
2F: Any Blue
3F: 0= Red
4F: 9= Green 9= Red
5F: 15 total Green
6F: 20 total Red
7F: 30 Any color
8F: 15 total Blue
9F: 2= Blue
10F: 3= Green 7= Red
11F (Twilight Town): 8= Blue 1= R 20 Green
12F (Destiny Islands): 33 of each color (RGB, or BRG in this case)
13F (Castle Oblivion): 30 Blue 40 Red 20 Green
The Room to Rewards and the treasures within
Now for those wondering what each Room of Rewards holds, this time, unlike the door requirements, IS world specific! Each room will have either one or two reward chests, depending on if you have fulfilled the small series of requirements involved.

The requirements for a second chest to appear in each world is to have watched the entirety of 358/2, read each entry of Roxas' Diary, read each Secret Report, and read each Character Entry (For note, you don't have to actually read everything if you don't want to, as long as it counts it as read, which should show the entry/movie blue it should count, exception being the character entries, those will just not say "New").

I am going to detail each reward for each individual world assuming that the bonus requirements have been met:

Warning beforehand, but if you open a room of rewards without the clear bonus, you will have to re open the door if you want the second chest!

Traverse Town:
First Chest: Lionheart
Second Chest (358/2 bonus): Saïx

Wonderland:
First Chest: Synchro
Second Chest (358/2 bonus): Xemnas

Olympus Coliseum:
First Chest: Metal Chocobo
Second Chest (358/2 bonus): Total Eclipse

Agrabah:
First Chest: Warp
Second Chest (358/2 bonus): Luxord

Halloween Town:
First Chest: Bind
Second Chest (358/2 bonus): Bond of Flame

Monstro:
First Chest: Aqua Splash
Second Chest (358/2 bonus): Xaldin

Atlantica:
First Chest: Quake
Second Chest (358/2 bonus): Demyx

Neverland:
First Chest: Thunder Raid
Second Chest (358/2 bonus): Midnight Roar

Hollow Bastion:
First Chest: Mushu
Second Chest (358/2 bonus): Xigbar

Twilight Town:
First Chest: Stardust Blitz
Second Chest (358/2 bonus): Roxas

Destiny Islands:
First Chest: Megalixir
Second Chest (358/2 bonus): Two Become One

Castle Oblivion:
First Chest: Super Glide
Second Chest (358/2 bonus): Star Seeker

I'm hoping my several hours of researching, suffering, and testing can help someone out with their own 100% of Chain of Memories, even if it isn't my favourite of the series, I can still appreciate it for what it is.

The cards after beating both stories (Ansem, Zexion, Lexaeus, Ultima Weapon) come from the Bounty room chests, not Room of Rewards chests. The Ansem card is on floor 11, Zexion card is on floor 12, and Ultima Weapon + Lexaeus cards are on floor 13.

Other notes:
* Maverick Flare is from a Bounty room in floor 1 (Traverse Town), but it requires watching 358 first.
* The Gold and Platinum cards (after 100%ing the journal's card collection) appear in Bounty rooms on floor 13.
