Ancestry Achievement: All QR Code Locations and Messages

Locations and transcriptions for all QR codes in The Talos Principle 2: Road to Elysium - Orpheus Ascending. Viewing all 12 will award the Ancestry achievement.

NOTE: As of now many players are reporting that you need to find all QR codes within one session for the achievement to unlock. It is uncertain whether or not this is a bug or intended.

Behind Chamber 15

In Chamber 15, build a staircase of boxes on the back wall, jump onto a pillar, and climb over the back. The QR code is on the outside wall. Turn left, and jump down onto the bridge to return.

Heart of Anubis

After solving Heart of Anubis, ascend the gravity well and turn left. The QR code is outside the room on the base of the structure.

What kind of encryption did they do to the "UNKNOWN ERROR" codes? Like how do you know the first one reads "The last enemy that shall be destroyed is death."? I've tried deciphering it myself but have so far come up empty handed
