Introduction
I originally didn't want to turn this into a guide since it's just a bunch of links, but someone suggested me to do it so it won't be lost easily, so here we go. This guide is basically the same as this post with some minor tweaks.

Since there are a lot of missables in the game, I figured it's better to list down everything you need to 100% the game. I know there are a lot of other 100% roadmaps, but I found most of them to not being optimized (requires at least 3 playthroughs despite you being able to do it in 2) and not that clear enough. This guide tries to make up for that.

Tips
While you play your first playthrough, make sure to:
  • do not play on easy
  • save in every save points you find
  • open at least 85% of the chests (just open as many as you can to be safe)
  • cook all recipes until you master them in 1 character except Repede
  • master 3 recipes with Patty for Amazing Chef title
  • scan every enemy (if you can see their HP when you hover them, it means they are scanned). Some enemies have different phases, you need to scan all phases (including those that you need to fight again after your party member changed - they usually have different entries).
  • equip Sephira or Blue Sephira to your party members to make it easier to get Piggybank achievement
  • finish 255 battles in Hard difficulty for Berserker title
  • get 100 Fatal Strikes to all characters (just turn on their Fatal Strike to Aim and change it to Do Not Use if that character already get it) for Fatal xxx title. If you have to farm this (i got it without having to do it), you can equip all characters with minimum damage weapon to make it easier
  • steal 100 items with Repede's thievery for Big Bad Bandit title
  • use Estelle a lot to heal (using her just for bosses should be fine) for Dedicated Paramedic title. You need her to heal 4000000 HP.
  • craft all items and weapons to register them to your item book
  • equip all weapon to learn the skills that you haven't learned, especially for Raven
  • trigger 200 blue fatal strikes and 1k fatal strikes (any color) with all characters for Judith's Into the Sky title
  • synthesize everything for your item book. One item is missable (Endless Heart) but it was already covered in the walkthrough I linked above.

Also, make sure you finish all secret mission in your first playthrough. The detail is covered in the walkthrough I linked above. Feel free to use item if you want, we can cover the Item Hater title in the second playthrough. Note that secret missions are replayable in Nam Combanda Isle which is available in the end game, so dont worry about missing them on the boss fight. Just make sure to follow the walkthrough I linked to the letter since 1 secret mission require 1 item from one of the side quests to complete.

Necropolis of Nostalgia
Nostalgia is Overrated
Make your way through the Necropolis of Nostalgia, and defeat the strongest enemies out there!

Coliseum

Do 100 man and 200 man with all of your party members, finish tag team tournament Heroes with all characters (need 2 attempts minimum). Also, you should get Karol's Chirpy Fighter title from his solo Coliseum attempt.

Pro Boarder
Finish the snowboarding mini-game in 40 seconds or less. You'll be a pro!


Do not accelerate all the time, try to get your muscle memory to get the idea how how much you need to tap. I used to alternate between accelerating and braking all the time. Grade grind
You need ~19k grade to get everything IIRC. There are several methods, but I used this to farm 5k overnight: https://www.youtube.com/watch?v=GBWgstFksDA


No More Grinding
Reach level 200. Push yourself to the max, and once you hit it, use herbs to grow even stronger!


You get some level up items from the free DLC. Use them until you reach lv 199. Use Patty's Critical Moment to get either x2 or x10 experience in the fifth layer in Necropolis of Nostalgia (Fatality) and explore the area a few times.
If your party is lower leveled, just use the same trick in Necropolis of Nostalgia to increase your level before using the DLC level up items.



Jackpot
Acquire 100,000 chips. Just be careful not to lose sight of your real goal...


Just save outside Nam Combanda Isle and play poker. Keep on multiplying the rewards until you got a ton of them. Winning a large amount once is better than winning several times with lower amount. Just be patient.



The Hit that Keeps On Hitting
Get a 150-hit combo. Must. Keep. Hitting!


The easiest one is to equip everyone with minimum damage equipments, use Rita and only activate Tidal Wave/Crimson Flare. Go to Halure and find a battle with a lot of enemies and spam Tidal Wave/Crimson Flare there with overlimit lv 1. Recast the overlimit lv 1 once it runs out and spam the Arte again until you reach the required combo. Change your weapons and kill the enemies afterward.



To Points Unknown
Travel 50,000 km. That's 1185 marathons!


To be honest, i got it without grinding, but you can just use AHK script (if you play with keyboard) or rubber band your controller (if you play with controller) and leave it overnight.



Bunny Guild Member
Play 5 hours with Yuri, Estelle, Rita, and Judith wearing bunny ears. Welcome to the Bunny Guild!


I coupled it with grade grind. Just have Yuri, Rita, Estelle, and Judith in your active party and equip them with bunny ears that you got from the guy in the Union. You need to have most of the titles to get everything from him (Yuri's bunny ears should be the last item from him). Once you do just AFK for 5 hours. Coupling it with grade grind or To Points Unknown is better to save time.
Second playthrough
Low Level Challenger
Defeat Barbos at level 15 or below. If you can pull this off, you can do anything! Well, almost...


Speedster
Complete the adventure quickly! Sometimes you need to relax and smell the roses. Just not now.


Note: you also can divide it into 3 playthroughs by choosing 10x exp in your second run and 1/2x exp in your third run. The strategy is still the same, except that you don't have to worry about reducing bonus exp to 0 in act 1.
  • Unlock everything. Make sure to activate Battle Experience and Unlock Every Skit. We aim for Low Level Challenger and Speedster in one playthrough.
  • Play in easy mode.
  • Try to avoid battles if you can, and get bonus EXP to 0 in Act 1. Do so by taking damage and spending battle for more than 25s. You can get some EXP, provided your lv is less than 14 (I finished Act 1 at level 10). Once you finished Act 1, you don't have to worry about bonus EXP, although try to not go into battle too often since it will affect your playtime.

Make sure you have the fell arms unlocked and have a high kill count with Yuri (I got ~8k attack with the fell arm equipped). It might not be needed to have it this high, but the higher your attack power, the easier it is to finish the game.

I finished the game in lv 40 ish, in ~5h. That's like half the time needed for Speedster (it requires you to finish the game in 11h IIRC).

Make save in the overworld once you unlock Ba'Ul, and once you finished the game, load that save and go to Nam Combanda Isle to view all skits you missed to get Character Study achievement and possibly Item Hater title if you still used items in Act 1.

Way too EZ! How do they give you 15 hours for the achievement? (Rhetorical Question). You only need like 10 and that gives you tons of room for error. I managed to clear this game in 7. Not even a challenge.

It will also serve as a great reference, people can look to see how to unlock a specific ach and read your advice and if they need more they can visit specific guides. I've made a few guides for symphonia like the casino coin ach which I thought was pretty tough and random and took me a while to figure out. Still random but there's some steps to make it easier.

Great guide sure it'll help out a lot of tales fans. Enjoyed the series since phantasia.

@Unseen: there is a 1/2 exp and battle experience (you can only choose 1 in ng+). Battle experience gives 1 exp after every battle but you get double bonus exp, so it isn't that hard (plus you can make up your low level with gears and easy mode) :)

The low level challenger looks like it might be kinda hard, unless there's a low xp option in the grade shop.
