Introduction
Hello! This is the first Steam guide I've created. Please let me know if I have any information wrong, or you have something to add!

Sunday
The first achievement you should get is Sunday, for simply starting the game! :]
Exploration Achievements
A total of 14 achievements can be acquired by simply exploring every tile, and collecting every item. These include:
  • Crossing the Rift (Explore 100% of Rift)
  • Harvesting the Rift (Obtain 100% of Rift Items)
  • And Here We See, Squares In Their Natural Habitat (Explore 100% of Natura)
  • These Aren't Natural At All (Obtain 100% of Natura Items)
  • Lord of the Rings (Explore 100% of Ring)
  • It's Done (Obtain 100% of Ring Items)
  • Hope You Used the Sonar (Explore 100% of Yorethera)
  • Under the Sea (Obtain 100% of Yorethera Items)
  • My Kingdom for a Map (Explore 100% of Rex II)
  • Tired Yet? (Obtain 100% of Rex II Items)
  • X Marks the Spot (Explore 100% of X)
  • Xcellent Work! (Obtain 100% of X Items)
  • Black Sheep Wall (Explore 100% of the Galaxy) (Secret)
  • What Even Is All This Stuff? (Obtain 100% of the Galaxy Items) (Secret)

This is extremely easy to do, and I would recommend that you do these achievements on your own. However, as this is a 100% achievement guide, all maps and items will be shown.

Rift

  1. Glasses (+1 Sight)

End Sequence Achievements
There are several achievements that can only be acquired when ending the game. They are:

You Are Achievement! (Secret)
Complete story mode. Pretty simple.

Not What Fasting Means (Secret)
Make it home in time for dessert. Complete the game in under 30 minutes. Do this one first, and then go back for any additional exploration tiles/items/achievements by replaying your story save file.

No Dessert For You! (Secret)
Take longer than 30 minutes and miss desert. Pretty easy, just replay your save to collect any missing items, or afk. Make sure to save before finishing, if you think your time may be too low.

You Are Asploded
Somehow die in this game. Just let time run out during the end sequence.

wow very save many animals (Secret)
Save the animals! During the end sequence, you will come across this blue door with some Doges behind it. Your items do not work during the end sequence, so it is seemingly impossible to open.
To open this door, simply continue along the path for a couple more tiles, until you hear the door opening sound, and the door will be open. Then, backtrack and collect the animals to acquire this achievement.

Look At Me, I Am the GOAT Now (Secret)
Achieve 101% of items. Make sure you have collected every item on every planet, and then begin the end sequence. Save the animals as you escape, getting "wow very save many animals" in the process. This will be 101% of items.
Game Mode Achievements
Big Brain Time
Complete all 40 puzzles. This one is pretty obvious; complete all puzzle levels.

Most puzzles will be covered in the Big Brain Time section below. If a puzzle is not listed, that is because I have not written a solution. If you would like me to, please ask, and I would be happy to!

Its Just a Bunch of Coloured Squares!
Complete all 40 Zens. Another pretty obvious one; just complete all Zentroid levels. Make sure to select the largest brush size in order to complete these as fast and efficiently as possible.

Gotta Go
Complete the game on Speedrun, any time is fine. Exactly as the description says, just complete the game in the Speedrun mode with any time.

Gotta Go Fast
Complete the game on Speedrun, under 10 minutes. Also exactly as the description says; complete a Spreedrun in under 10 minutes.

You Can't Even See The Trophy (Secret)
Complete the game in Moletroid. Moletroid completely removes your ability to see the map, and any unlocked squares besides the one you are currently on. I would recommend having a map open for this, or just brute force your way through it. At least make sure you have a rough understanding of where all the key items are before attempting this.

Return of Squaremus (Secret)
Play in full GB mode. Select both the menu and map GB alts from the alt menu. This achievement will be granted immediately after you select both.
Big Brain Time
To get this achievement, you have to solve all 40 puzzles. These are not terribly difficult, so I would recommend doing them all on your own, without help. The only puzzle that gave me any trouble was 40. "Solve for M". However, I will be giving solutions here anyways, so please skip this section to avoid spoilers!

1. Crates
To solve this puzzle, you have to move each of the three crates onto their own dots. Crates can only be pushed, not pulled, so if you get a crate stuck, you must reset.

2. Dial M For Maptroid
This puzzle is just like the popular puzzle type of "picross". To solve these puzzles, you have to paint a number of consecutive tiles in a row or column, depending on its number. If a row or column has multiple numbers, that indicates at least one space between a different groups of tiles.

3. Going Up
To solve this puzzle, move your icon until it is next to the middle row of empty squares. This will cause the top square to slowly come down, stopping and playing a ding sound once it reaches the bottom. Then, step inside the tile, and it will move back up, and you wlll be able to reach the exit.

4. Eat Apples
This puzzle plays similarly to the popular arcade game "Snake". The only difference being that your character does not automatically move. To solve this puzzle, simply move towards the apple icon, and don't cross over yourself!

5. Amazing
This puzzle functions similarly to the base game in story mode. As you move, tiles around you are revealed, and stepped on tiles become colored. To solve this puzzle, simply reach the end.

6. Follow The Path
To solve this puzzle, simply follow the path shown on the top of the screen. If you mess up, you can undo moves by backtracking your path.

7. C-C-C-Combo Breaker
This puzzle also has the hints "I love pie" and "Pies are circles" in it. These are referencing mathematical pi, 3.14. To solve the puzzle, use the + and - buttons to change the three digit display to "314". This will open the door, and allow you to move to the finish.

8. Think Outside The Box
To solve this puzzle, move out the gap, and around the outside to the other gap, and reach the finish.

9. A g
This one is similar to "Amazing", just that the maze is larger and flashes into existence. To solve this puzzle, just move around, and wait for the flashes to determine where to go. I have included the full, lit up maze as well.

10. Mirror Mirror
This one is also just like "Amazing", except the twist is that you also move a mirrored character in a mirrored room. You control the left character, so just focus on solving the maze with them.

11. Fall Block Fall
This puzzle is a reference to the popular arcade game Tetris. Somewhat disappointingly, you do not actually play Tetris. To solve the puzzle, all you have to do is press your interact button, probably spacebar, and wait for the block to move to the bottom. That's it. Probably the easiest puzzle of them all.

12. Off And On Again
To solve this puzzle, step on the top left dot, and the bottom right dot. This will open both doors, and allow you to reach the exit.

13. Broken Mirror
Yes another level similar to "Amazing". This time, the twist is that the maze is invisible, and a reflection of the visible maze to your right. To solve this puzzle, traverse through the maze with your guide, and keep in mind that it is a reflected copy.

14. Run!
To solve this puzzle, press the red button on your left, and then spam your right movement key to reach the end before the door closes after 3 seconds.

15. Behind You
To solve this puzzle, move into the hidden room to the left of you. This will reveal a crate. Push the crate onto the button, keeping the door open, and allowing you to reach the exit.

16. Patience
Ok, maybe this one is the simplest puzzle. All you have to do is nothing for 5 seconds, and then just move onto the goal.

17. Colored Squares
This one I found to be slightly difficult. To solve this puzzle, move all the squares of one color onto the middle square. It is ok to have leftovers, just fill the middle with any color.

18. Red Light Green Light
To solve this puzzle, you have to wait for the "stoplight" to reach the bottom green square before you can pass the intersection and make it to the goal. If you do not wait before crossing the intersection, you will fail and the level will restart.

19. Ready Set Go
To solve this puzzle, you have to make it to your goal before any of the other colored cursors make it to theirs. If you are too slow, you will fail, and the level will reset.
Big Brain Time (Continued)
I did not know this, but Steam guides have a character limit per section! More puzzle solutions are below.

20. Opposites Attract
In this puzzle, you have to get the white cursor onto the dot, but the orange cursor mirrors your movements and blocks you. To solve it, move the orange cursor into the top right as shown, and then you will be able to move to the white dot.

21. Draw Swords
Another Picross level. Fill it out as shown below.

22. Fast Apples
Another Snake level. This time, you automatically move forwards. Simply redirect yourself to continue eating apples until you win. It's a little hard to tell, but I believe you get faster with each apple you eat. Also, you can tap the direction you are currently moving to move faster, if you feel too slow.

23. Falling Apples
In this level, apples fall from the sky continually, and you control the bucket at the bottom of the screen. To win, move your bucket under these apples and let them fall into the bucket until you win.

24. Square Square Revolution
This level is a take on "Dance Dance Revolution", a popular dancing rhythm game played in arcades. To win, press the movement key that is in the direction of the dot on the screen, in relation to the cursor, as many times as needed.

25. Map Moves You
This level is a silly variant of the maze levels. Your cursor stays in the middle of the screen, and instead the map moves around it. To win, move the dot so that it is on your cursor.

26. Amazen't
In this level, you are navigating a maze to the dot, except there is only one path, and you can not see it. The path is shown in the solution below.

27. Hadouken
This level is simple. Move your cursor onto the dot, and press space to win. Hadouken is a reference to a move in the fighting game "Street Fighter". I didn't google this information so I'm guessing. Correct me if I'm wrong.

28. Sustroid
This level is a reference to the game "Among Us". To win, move your cursor to each white dot as it appears, and avoid touching the red cursors. They move at random intervals, but in straight lines back and forth.

29. Sunk My Battleship.
This level is a reference to the board game "Battleship". To win, interact on the corresponding red tiles show in the solution, which would normally be found randomly. Remember, your side is a mirror of the other, so moving right moves your attack left.

30. Ghost Leg
To solve this puzzle, move in the way shown below. If you ever cross over a red square, you will need to backtrack to correct this mistake.

31. Cannon
In this level, you are trial and erroring to find the right angle and power to launch the dot into the bucket, The left and right arrows control power, and the top and bottom arrows control angle. Interacting with the middle fires the dot. I'm sure there are many solutions, but one that works is 26 angle, and 45 power.

32. Moletroid Memory
This level is similar to the Moletroid gamemode, in that you can only see your current tile and any given time. The solution to this maze is the same as Amazen't. Just figure your way around if you remember that solution.

33. Random Encounter
This level is trying to emulate a jrpg's turn based combat system. To win, you have to reduce the Slime's bar to 0. Using "Fight" deals 1 to you and the Slime. Using "Magic>Fire" deals 1 to you and 2 to the Slime. Using "Magic>Poison" deals 1 to you, and heals the Slime for 1. Using "Item>Tonic" heals you for 1. And lastly, using "Run" starts the encounter over. Just deal damage with Fire, and won't need to heal to win.

34. Woof
Another Picross level. The solution is shown below.

35. M-ovetroid
In this level, you are moving horizontal rows to make the M in the logo. To win, move each row until it looks like the below solution.

36. Minesweeper
This level is a take on the computer game "Minesweeper". In this game, each tile is surrounded by numbers, indication how many cells adjacent to the number itself contain mines. So, a "1" means that in all the tiles surrounding it, 1 and only 1 of these tiles can contain mines. I won't go into detail about Minesweeper theory here, and you likely already know how to play Minesweeper anyways. To win, open all non-mine tiles, or open the tiles shown below.

37. Shutdown
This level acts like a Zentroid level, in that tiles get revealed in a radius around you. To win, navigate towards the top right, put your cursor on the "X", and then press interact.

38. Squaredoku
This level is similar to the logic game "Sudoku". In these types of games, you have to fill in the rest of a partially filled board, so that no number in any row or column repeats, or that any number in (typically) a 3x3 box that is drawn on the board repeats. In this version, the boxes are 2x2. To win, interact with each red square to increase the number by 1, and solve the Sudoku as shown below.

39. X to go
This level is during a game of tic-tac-toe. To win, you have to create a three in a row of X's, using the currently filled board. Place the X in the square shown below.

40. Solve for M
The trickiest puzzle in the entire game. The mechanic of this puzzle, is that you control the white square, and every time you move into a square, it swaps places with you. Your goal is to create a letter M as pictured. Note that the bottom right tile does not exist, and does not need to be filled.

I have also taken the time to write out exact directions to solve this puzzle. It is very long, and I know for a fact that it is not optimal. However, if entered exactly as written, you will solve 40, without even needing to look at your screen. If you can come up with a better sequence, please tell me, and I will update the guide, and credit you.

Huginn? What's the answer to number 40? I've tried solving it, it's really hard to align it.
