Baldur's Gate 3 is a massive game with many quests that can be easily missed. In this guide, I have compiled a checklist for those who would like to ensure they haven't missed any content. I created this guide based on the game's official wiki.

Introduction
First of all, English is not my native language, so I apologize for any grammatical errors.

Obviously, this guide will contain spoilers, so if you don't want to, avoid using this guide on your first playthrough.

This guide is not a step-by-step walkthrough, so you won't find exact information on where to locate the quests and how to complete them. Instead, it's a list to help you identify those you may have missed, just like I did. However, I've also provided a link to the Wiki[bg3.wiki] for each quest in case you need more details about it.

Disclaimer: I only included the beginning of quest chains. If you can't find one, please make sure it's not a subquest of any of the already listed quests.

I'm aware that there are companion quests that are automatically added to your journal, but the purpose of including them in the list is to explain how to trigger them.
Prologue - The Nautiloid
Quest
How to start
Takes place immediately after the opening cinematic.
It can be started by approaching Shadowheart's Mindflayer pod on the Nautiloid.
Act 1 - Ravaged Beach
Quest
How to start
Free Lae'zel[bg3.wiki]
It is initiated by finding where Lae'zel is being held in a cage by a pair of tiefling scouts.
It can be started by approaching the ruins north of the Overgrown Ruins waypoint, or by reading the Disintegrating Journal[bg3.wiki].
It can be initiated as soon as you rescue Lae'zel on the Over-world and have her join your party.
Druid Grove
Quest
How to start
It can be started by speaking with Zevlor or Rath.
It can be initiated by speaking with Aradin about the reason for his expedition into goblin territory, or coming across the corpses of the failed expedition.
It is initiated by going to the Druid Grove prison and intervening during the confrontation between Arka and Sazza.
It can be initiated by speaking with Sazza at the Druid Grove.

Hunt the Devil[bg3.wiki]
It can be started by speaking with Wyll at Druid Grove or a group of paladins of Tyr in a Toll House at The Risen Road.
It occurs by browsing the wares of Mattis. (There's a sequence of 3 perception checks before this mission starts. The first one doesn't matter, but you must fail the second check and pass the third one to begin this quest.)
It can be started by speaking with Barth or Meli at the Druid Grove. (If Aradin and his party are at Druid Grove.)
It can be started by approaching the Secluded Cove at the North-Eastern region of the Druid Grove.
Save Arabella[bg3.wiki]
It can be started when nearing the Sacred Pool. Kagha, acting leader of the druids, is holding a tiefling child captive.
It can be initiated after entering the Druid Grove by speaking with various Non-player characters about Halsin's disappearance.
It can be started by inquiring about a healer with Nymessa and Damays or various individuals at the Druid Grove.
In order for Gale to open up, he needs to have medium affection with the origin character and trust them. Several actions can lead to Gale placing his trust in the character, such as defusing tempers among Aradin and Zevlor when entering the Druid Grove for the first time, saving Arabella from the snake in "Save Arabella," rescuing Mirkon in "Investigate the Beach," refusing to let Arka shoot Sazza in "Save the Goblin Sazza," agreeing to take the Wyvern Poison after Nettie poisons the party in "Get Help from Healer Nettie," and successfully following Gale's protocol if he dies.
To receive this quest, the party must have established a positive relationship with the tiefling children in the Grove. Several quests can impact how Mol perceives them: "Investigate the Beach", where they must save Mirkon from the Harpies; "Save Arabella", which involves rescuing Arabella from Teela; "Find Your Belongings", where the party must refrain from threatening the tiefling children; and "Return the Locket", which requires resolving the conflict with Meli and returning the locket. (It's not possible to start this quest if you finish "Investigate Kagha", "Save the First Druid" or anything that decides the grove's fate prior to talking with Mol.)

Find Doni[bg3.wiki]
It can be started by speaking with Rikka at the Druid Grove. (Find Doni only occurs if the leaders of the goblin camp are left alive and learn the location of the Druid Grove. If you saved Sazza or told Minthara the location.)
Blighted Village
Quest
How to start
It is initiated by discovering a group of goblins in the Blighted Village laughing at having tied Barcus Wroot to the blade of a windmill.
It is initiated by finding a ledger in the apothecary within the Blighted Village that mentions rare plants stored in the cellar.
It is initiated by picking up Highcliff's Journal in one of the houses in the Blighted Village or by looting Highcliff's Blueprints in the basement of the same house.
Find infernal iron in the basement of the same house where you find Highcliff's Blueprints. (Take it to Dammon at Druid's Grove with Karlach in your party.)
Sunlit Wetlands
Quest
How to start
It can be initiated by speaking with Auntie Ethel at the Druid Grove, or running into her south of the Blighted Village.
Save Mayrina[bg3.wiki]
The quest is acquired by running into an engagement between Auntie Ethel and Mayrina's brothers - Johl and Demir, south of the Blighted Village.

The Risen Road
Quest
How to start
Speak with Karlach and recruit her to your party.
A burning tavern is surrounded by the dead bodies of several Goblins, Gnomes, Drow and Flaming Fist mercenaries. Help the Flaming Fists to open the gate. If you fail, you can attack the door.
It will be initiated once you come across the trapped Benryn at the top floor of the burning inn at Waukeen's Rest, during the events of the quest "Rescue the Grand Duke".
It can be started by approaching the trapped Rugan and Olly at the Risen Road, finding the Zhentarim Basement and speaking with Zarys there, or reading a note found on the corpse of one of her missing agents.
Free the Artist[bg3.wiki]
It is started by speaking with Oskar Fevras at the Zhentarim Basement in Act One and does not resume until Act Three in Baldur's Gate.
Goblin Camp
Quest
How to start
Rescue Volo[bg3.wiki]
It is initiated as soon as you enter the area and engage with Volothamp Geddarm, who has been captured and is forced to perform on a platform.
Underdark
Quest
How to start
It can be initiated by speaking with Blurg and Omeluum at Ebonlake Grotto (the Myconid Colony), and informing them of the parasite.
It can be initiated by speaking with Thulla at the Myconid Colony.
It can be initiated by speaking with Thulla at the Myconid Colony, after completing "Cure the Poisoned Gnome".
It can be started by talking to Derryth Bonecloak at the Myconid Colony.
It is initiated by speaking with Gekh Coal at the Decrepit Village.
It is initiated by learning of the forge of the same name by reading The Great Furnace of Grymforge, Filro the Forgotten's journal at Dread Hollow, or speaking with Dhourn after freeing him of the Spectator's petrification or casting Speak with Dead on his corpse.
It can be initiated by approaching the Myconid Colony at Ebonlake Grotto peacefully, and speaking with Sovereign Spaw.
It can be started by talking to Glut in Myconid Colony.
It can be started if the player talks to Sovereign Spaw at Ebonlake Grotto, after completing Defeat the Duergar Intruders.
Grymforge
Quest
How to start
It can be started when the player arrives at Grymforge and talks to Sergeant Thrinn.
Rescue Wulbren[bg3.wiki]
Once you've successfully completed the quest "Save the Grymforge Gnomes" and discovered that the other gnomes were taken to Moonrise Towers. Otherwise it can start when encountering Wulbren in the Moonrise Towers Prison.
In a locked Adamantine Chest, on an island in lava, near the Lava Elemental in Grymforge (X:-624, Y:224). Proceeding to talking to the amulet progresses this quest to "Help the Cursed Monk".
It can be initiated by talking to Halsin or Minthara, depending on the player's choice and outcome of the quest "Save the Refugees", after each branch's respective celebration at the Campsite.

You can go to the mountain pass to explore and progress to Act 2, but you can return at any time and choose the Underdark pass. It's possible to go back freely until a certain point in Act 2.
Mountain Pass

How to start
It can be started by speaking with Lady Esther at the Mountain Pass.
It can be started by a note from a Lathander worshipper regarding a precious relic called "The Blood" hidden somewhere in Rosymorn Monastery.
Act 2 - Shadow-Cursed Lands
Quest
How to start
It can be started by speaking with Goblin near the entrance to the Shadowlands from aboveground in the Mountain Pass.
It can be started by approaching the elevator in Grymforge or entering the Shadow-Cursed Lands.
IIt can be started by speaking with Halsin after entering the Shadow-Cursed Lands.
Ruined Battlefield
Quest
How to start
It can be started by reading the note in the Potter's chest in the Ruined Battlefield.
It can be started by speaking with He Who Was in the Ruined Battlefield.
Moonrise Towers
Quest
How to start
It can be started by speaking to Z'rell or Ketheric Thorm in Moonrise Towers.
It can be started by entering Moonrise Towers if Minthara survived the previous act. It is not required to have raided the Druid's Grove, but if the player knocked her out to progress in "Defeat the Goblins," the game will continue as if she were dead. (You can't have Halsin and Minthara in the same run.)
It can be started by speaking with Thisobald Thorm in The Waning Moon, by speaking to Disciple Z'rell in Moonrise Towers, or by reading Balthazar's diary in his chambers in Moonrise Towers.
It can be started by completing "Infiltrate Moonrise Towers".
It can be started by speaking to Wulbren in Moonrise Towers at the end of Act Two or by approaching the gate to Wyrm's Crossing in Rivington. (Only if you completed "Rescue Wulbren".)
Last Light Inn

The Pale Elf[bg3.wiki]
Speak to Raphael in the shadow-cursed lands while having Astarion in your party.
It can be started by speaking with the tiefling children at the Last Light Inn. (Only if you sided with the tieflings).
Find Zevlor[bg3.wiki]
It can be started by speaking with the tieflings at the Last Light Inn. (Only if you sided with the tieflings).

It can be started by speaking with Fist J'ehlar at the Last Light Inn.
Reithwin Town
Quest

It can be started by speaking with Arabella in Reithwin Town. (Only if Kagha didn't kill her.)
Grand Mausoleum

How to start
It can be started by speaking with Raphael near the entrance to the Thorm Mausoleum.
Explore the Gauntlet of Shar and complete the Trials of Shar.
Mind flayer colony
Quest
How to start
Confront the Elder Brain is acquired at the end of Act Two, after the Chosen Three are revealed to be controlling the Elder Brain as part of their plot to take over Baldur's Gate. (However, the quest cannot be meaningfully progressed until "Get Gortash's Netherstone" and "Get Orin's Netherstone" have successfully been completed.)
Act 3 - Rivington

It can be started by speaking with Sister Yannis in the Open Hand Temple.
It can be started by completing "Solve the Open Hand Temple Murders" provides more information on Father Lorgan, the latest victim of the Bhaalist plot, as well as information on the killer behind the murders.
It can be started by speaking with Ulma in Rivington or the vampire spawn at Fraygo's Flophouse.
It can be started by speaking with the starving mind flayer in Rivington.
It can be started by speaking with the Mamzell Amira in Sharess' Caress.
It can be started by speaking with Farlin in Rivington or by visiting Sharess' Caress.
It can be started by speaking with Lucretious in the Circus of the Last Days.
It can be started by reading the Blackmail Letter in the basement of Arfur's Mansion.
It can be started by speaking with Manip Falcäo in Rivington.
It can be started by speaking with the Strange Ox in Rivington. (Only if you didn't kill it.)
It can be started by speaking with the Danzo Arkwright at Sword Coast Couriers.
It can be started by approaching the Wyrm's Rock Fortress gate from Wyrm's Crossing.
It can be obtained by speaking to either Mamzell Amira or Korrilla in Sharess' Caress.
The High Harper[bg3.wiki]
It is added to the journal if Jaheira joins the party towards the end of Act Two and the player speaks to her once they arrive at Wyrm's Crossing.
It can be started by speaking to Enver Gortash in the audience chamber above Wyrm's Rock Fortress.

It can be started by speaking to Helsik in Devil's Fee.
It can be started by speaking with Rakath Glitterbeard in the Counting House.
It can be started by speaking with Allandra Grey in the Lower City.
It can be started by speaking with Zanner Toobin in the Steel Watch Foundry.
It is triggered when talking to Blurg in The Lodge or is automatically triggered upon visiting the Iron Throne during "Save the Gondians" if Lady Esther was not given a githyanki egg in "Steal a Githyanki Egg".
It can be started by speaking with Estra Stir in the Lower City. (The latter half of this quest is trivialized if "Disable the Steel Watch" has been completed.)
House of Hope

Just added 'Explore the Ruins' in the Act 1 section. It’s been a while since I wrote this guide, not sure why I didn’t include this quest before.

You can also get "Retrieve Omeluum" by talking to Blurg in The Lodge in the lower city.

Help Alfira with Weeping Dawn Song – If you choose to use her mentor's instrument, the reward will include the instrument and teaches any class character how to play.
Save single Teifling from Bugbar – hillside to the right of the main gate. Rewards a soul coin.
Help Teifling in shack (lesser restoration spell for paralysis)
Return Locket to Bart (variations on ending depending on how you speak to the pickpocket)
