Introduction
Cavity Busters is a difficult bullet hell roguelike that currently has 31 achievements. A lot of these achievements are easy to get, but some require a bit of grinding. If you are looking for information on a specific achievement or unlock, use 'CTRL+F' to search for it quickly.


This guide contains spoilers!

Unmissable Achievements
You shouldn't worry about these achievements as they are achieved by playing through the game normally.


One set of teeth
Get 100 kills
100 enemy kills, you'll get this pretty quickly.


Six sets of teeth
Get 600 kills
600 enemy kills, it'll take a bit longer.


Ten sets of teeth
Get 1000 kills
The last achievement relating to number of enemies killed, you'll achieve this over time.


Just a cleaning
Beat the game
Defeat the final boss in Ortho II and go through the opening at the top of the room.
Easy Achievements
These achievements are straightforward and have common circumstances to unlocking them.

X marks the spot
Find a secret hole
Sometimes upon entering a secret room, a paper note will appear in the center of the room. When walked over, it will reveal directions to another secret, unmarked part of the map. Each dash represents a room and the direction they are in. The starting point of the map is not in the special room of which you first found the note, but after it. At the destination, dig up the secret hole to get this achievement. This may require a few shovels to get, depending on the map generation. Remember, you can sacrifice health to dig a wall.




Bestiary
Find your first bestiary entry
Uncommonly, an enemy may drop a slow-spinning polaroid. Upon touching it, the player will be rewarded with their bestiary drop.
More information on bestiary drops can be found later in this guide.




Harvest Tuum
Have a full tuum garden
The Tuum Garden is unlocked after defeating the final boss at least once.
While in the hub, go straight downwards and you will enter the Tuum Garden. In order to utilize the garden, you will need to plant seeds. You will always get a seed after beating the final boss, and they can also be uncommonly found in special rooms. Simply have 6 seeds and press 'E' on each patch to plant them to get this achievement. Note that when you plant a seed, it will choose a disease and give you it at the start of every run. The seed will then have a certain requirement, like a certain number of rooms explored or flawless boss kills to harvest the seed and remove the disease from your runs. Harvesting the seed will give a few stackable upgrades or a full upgrade as a small boost in your next run.
You can prematurely remove the seeds by using a scythe (found sometimes in special rooms, along with a watering can). Using a watering can will fill the seed's harvest requirement by a certain amount, effectively acting as a weaker scythe.


Intermediate Achievements
These achievements are slightly more tricky to obtain.

All the frogs in your throat
Take every disease in a single run
There are many diseases in the game where the harmful effects outweigh the benefits, which can easily end your run. However, many diseases tend to increase DPS quite a bit, which can be beneficial. Try to speedrun the game and rush to the boss room every time to encounter as little diseases as possible. If you are forced to take a really bad disease, use the doctor to cure it (if applicable).


No frogs in your throat
Complete a run without taking any diseases
The opposite of All the frogs in your throat. Although this achievement is slightly easier due to it avoiding the unpredictable nature of diseases, it is still recommended to speedrun through the levels, as leaving a lot of diseases behind will make the game significantly harder. This achievement can also be a cakewalk if you find the "Mask" item, which removes all chances for diseases to spawn for the rest of the run.


Shard it
Release all shard enemies from their calcified prison
Uncommonly, a strange yellow crystal shard will appear in the center of a special room. Attacking it will trigger a boss fight where the crystal will fire a sequence of regular bullets followed by a small amount of parry bullets. As the crystal is completely immune to regular attacks, the only way to damage it is by parrying its projectiles back at it until it falls apart. When it's destroyed, it will unlock a new shard enemy that acts similarly to the boss (ergo, it can only be killed by parry bullets) and can spawn in various rooms. Currently, there are 5 different shard enemies. Meaning you must find and defeat five shard crystals to get this achievement.


Jaws of life
Get resurrected twice in a run
After defeating a boss, you can feed crowned teeth to Gob. If Gob has 4 or more teeth, he will offer to resurrect you mid run at the cost of 4 teeth. Because Gob can have up to 8 teeth at once, you can feed him crowned teeth until he's full and use 8 teeth to resurrect twice in one run. You can also earn and feed him crowned teeth mid run and resurrect twice that way. The most efficient way to earn crowned teeth is by flawlessly defeating bosses (not getting hit once).


Tooth In The Stone
Extract Jacks tooth
Jack is a special character that may appear uncommonly in special rooms. He offers to lower the game's difficulty in many ways in exchange for fillings or crowned teeth, with the latter having a greater effect. The easiest way to get this achievement is by giving him lots of crowned teeth at once, after which, the sword stuck inside of him will fly out and change his appearance permanently.
Hard Achievements
These achievements are difficult and time consuming.

Cavity Filled
Beat the game on hard mode
Hard mode is an alternate game mode that can be turned on in the hub in the green computer terminal to the left. It is unlocked by default. All enemies now have more difficult attack patterns and have a higher chance of becoming champions. Bowel movements also become more common. This is a difficult game mode that needs practice to be completed, but it can be made easier with positive modifiers also found in the terminal. It will still count towards the achievement.

Dig 30 times in a single level
This achievement is best done by accumulating many shovels throughout the run and eventually choosing a floor to splurge them all on. Loop if needed to get more resources.

Filling (A SHINY Gummy)
-Shotgums are replaced with bullet clearing bombs
-Jumps and wall run replaced with a wall slam

Unlock: Donate 999 fillings to the shopkeeper.

Tongue
-Tongue charge deflects non-yellow bullets
-Lower parry damage
-Attack power ups to change them
-No contact damage

Unlock: Deflect 700 bullets and get 700 shotgum kills.
TIP: An easy way to grind shotgum kills is by getting kills as Filling gummy. Because all of Filling gummy's bullets are considered shotgums shots, every kill counts as a shotgum kill. You can check the amount of shotgum kills you have by going to the stats page (ESC -> Collectibles -> Stats).
TIP: Grind to 1000 shotgum kills, as it's required to unlock a certain item.

Baby Teeth

Unlock: Occasionally, there is a secret special room that is far away on the map. Upon reaching it, there will be a bottomless pit that you can toss items into. Toss some items in. Upon reaching the same room later in the run, you can destroy the sacs in the room, unlocking Spider Gummy.

Tooth Gummy

TIP: Tooth gummy is possibly the hardest character in the game to use. Beating the game as Tooth Gummy with helpful modifiers will still give Tooth Win achievement.
TIP: After unlocking Tooth Gummy, you can redeem your reward from Gob, which will be a random buff that lasts for 2 runs. Afterwards, you can give Gob some of his teeth back to keep the special buffs he gives.

Full Body Cleanse
Achieve a 100% body cleanse.





SHMUP Achievements
At the end of every level, Gob (the thing that takes you to the next level) can be given crowned teeth that will in turn give the player various buffs. Giving Gob a full set of teeth will allow you to access the secret SHUMP levels when transitioning to a new floor. There are three bosses combined in all of the levels, including Gob himself as the final boss.

You can earn crowned teeth from flawless boss kills, having a high combo meter and sometimes in secret rooms.

Clear a shmup level without getting hurt or letting an enemy escape
The easiest way to get this achievement is by doing the first SHMUP level after the first boss. This level is by far the easiest, and you can easily retry it due to it only being after the first boss.

Fully Body Cleanse
Achieve a 100% body cleanse
After beating the final boss of the game and Gob has all his teeth, you will fight Gob himself. After defeating him you will enter a cleansing mode where you have to shoot down every item in the game in the same SHMUP style. When destroying an item, it adds on to a percentage at the top right part of the screen. This needs to be at 100% to get the achievement (meaning every item must be destroyed). This achievement will also unlock the Tooth Gummy.
Modifiers
Modifiers change the game in a easier or harder way. Unlocking all the modifiers is required for 100% completion. There are also a few achievements related to beating the game with certain modifiers. You can still earn achievements while modifiers that make the game easier are turned on.

Most modifiers are unlocked by defeating the secret boss Dronk over and over again.

How to unlock all modifiers:


Dig Mode:
Dig 175 times across multiple runs. You'll get this without trying, as digs add up overtime.
Hard Mode:
Unlocked by default.
Alt Hard Mode:
Defeat Gob in Hard Mode(?). See SHMUP Achievements
Champions Only:
Defeat Dronk.
Pure White Teeth:
Defeat Dronk.
Busting of Isaac:
Defeat Dronk.
Turbo Mode:

Shorter Run Mode:
Achieve 100% body cleanse. See SHMUP Achievements section.

Sad Boy 2020
Beat the game with a modifier enabled
Beat the game with Busting of Isaac modifier enabled.



Tooth Champ

Pearly White
Beat the game with a modifier enabled
Beat the game with Pure White Teeth modifier enabled.



Secret Bosses (Spoilers)
The game has a few secret bosses that are necessary to 100% the game.


Dronk
How to find: Dronk is always found on the first level of the first floor in the run (and on subsequent loops). On the mini map his room has a gray background with a yellow circle in the center.

-Upon entering the room, there are 4 different computer screens that need a code inputted.
-The number of special rooms can be easily seen by holding the tab button or jumping.
-You'll need to remember the amount of times you've taken damage on the current floor (on looped runs, it takes into account the amount of times you've taken damage throughout the entire run, making fighting Dronk on loops difficult if you don't keep track).
-The bottom right code is always 20.
-Ideally, the code for the random number is inputted last.

After everything is inputted correctly, Dronk will spawn. Defeating him will unlock one of several available game modifiers that can be turned on at the green terminal in the hub.

Diseaple
How to find: Diseaple can appear randomly in special rooms and will always offer the player a few diseases. To actually fight him, you will need to have the mask item. This will anger Diseaple, and the fight will begin a few seconds after.

Metal Gum Flacid
How to find: MGF can be found uncommonly in special rooms, sleeping. Upon entering his room, he will awake a few seconds later and the fight will begin. MGF will always drop a polaroid of a non-boss enemy on death.

None, but MGF is integral to efficiently farming bestiary polaroids.





SHMUP Bosses
How to find: Three separate bosses. See "SHMUP Achievements" section. None of these bosses drop polaroids.

Shopkeeper
How to find: The shopkeeper sometimes spawns in the floor in their shop, selling various items. To fight the shopkeeper an item will have to be stolen. Go over an item and hold 'E' for a bit and then let go. Do this repeatedly until the item is stolen. Afterwards, leave the shop and explore a few rooms. The shopkeeper will randomly confront you and the fight will begin. If the shopkeeper hits you with a projectile, he will steal an item from your inventory. The shopkeeper will attempt to steal around the same amount of items you stole from him.



Related Achievements:
None, but there are a few items that are unlocked by stealing/buying items from the shopkeeper.


Tooth Caps
Tooth caps change the way your projectiles look. They can be equipped by interacting with the NPC to the right at the hub. Tooth caps are purely cosmetic.


Tooth caps are unlocked by defeating the final boss or Dronk over and over again. Chances are you'll collect all the tooth caps without noticing, as they are very easy to unlock.

Synergies
A synergy is a combination of specific items that will give you a special buff. Some synergies require diseases and regular items in them. there are currently 19 synergies in the game.

Synergy Farming
Attempting to obtain synergies purely by chance and playing normally can be too daunting and grindy, there are easier ways to find them. The main method to obtain synergies is by grabbing as many items as you can. Because you cannot have two of the same upgrade/item (non-stackable upgrades) in the game, you can simply exhaust the item pool enough to the point where it's forced to give you the items you want.

To efficiently grind synergies, the following modifiers should be turned on.
Busting of Isaac - Double upgrades = item pool is exhausted more quickly.
Turbo Mode - Speed up the game, speed up the grind.
Chill Mode - Increased damage.

Try to avoid:
Shorter Run Mode - Making your runs shorter is a bad idea in this case, as it could force you to loop. In loops, items only spawn after defeating a boss, and diseases become extremely scarce (unless you have Dirty Tissue), making the grinding process take longer. If the synergies you're after don't require diseases, looping is fine for the most part. However, it will take longer overall, because the maps become much larger and the boss rooms are more hidden.

Method 1: Toy Chest
By giving Gob a full row of teeth and redeeming it, he will reward you with a powerful buff that will last a few runs (example: Vaccine, cures all diseases (if applicable) that you pick up for 2 runs).
One such reward is the Toy Chest. The Toy Chest will spawn a circle of items around the player every floor and allow them to choose one item, causing the rest to despawn. This buff even works in loops, meaning you will eventually get most items in the game and thus, most of the synergies.
Keep in mind however, the Toy Chest does not normally spawn diseases.

Method 2: Looping
Given that obtaining the Toy Chest buff is based on luck, it may be preferable to simply loop multiple times. It's important to note that you should collect the diseases you need before looping, as diseases are hard to obtain in loops. Collect every item you see (with some exceptions) to decrease the item pool.

Extra tips:
Helpful items:
Traveling Merchant: Sells an item every floor.
Mr Morph: Chance to spawn an item changer that can reroll every item in the current room up to 3 times in exchange for health. Extremely useful if you break open a lab experiment in a special room with Busting of Isaac enabled.
Creutzfeldt-Jakob (disease): Only take this early on. Resets all your upgrades, but makes it so there is a 1 in 10 chance an upgrade spawns every room clear. With Busting of Isaac, two items or diseases spawn if it procs.

Bad items:
The Mask or Dirty Tissue: These cause regular items or diseases to never spawn again, which makes collecting most synergies impossible.
Jacks Tooth: Although you'd want to collect every item you see to thin out the item pool, this particular item sets your hp to 1 every time you fire, which can end your run in an instant.
Glassy Stare (disease, no cure): Makes it so enemies have a lot less hp but in turn are invulnerable for 5 seconds upon entering a room, including enemies spawned by other enemies. This disease will just grind your run to a halt and make it insanely slow to traverse the map (especially on loops!).
Spikey Hat (disease, no cure): Adds more secret rooms to your run but makes the minimap have much less information. Makes it much slower to move around the floor.
Vague/Odd Item Unlocks
When hovering over a locked item in the collections page, it will tell you how to unlock said item.
However, some items have vague or confusing unlock hints.
This section will go over how to unlock certain items that have an vague phrase or '???' as their unlock hint.
You can view all of your locked and unlocked items by doing: Esc -> Collectibles

The location of these items in the collection screen will be determined by this parenthesis:
(row, column), under the section of their respective page number.

Pop 5, 20, 50, 100 zits (items unlocked this way span across pages 1 and 2)
Unlocks:
Wall Pores (row 1, col 2) (5 zits popped)
Boils (row 2, col 6) (20 zits popped)
Poopy Zits (row 1, col 10) (page 2) (disease) (50 zits popped)
Pimples (row 2, col 7) (page 2) (100 zits popped)

Zits commonly appear as red balls along the walls of a room. To pop them, simply initiate a wall run and pass over them. Some rooms may have a modifiers where the walls are full of zits.

A room that was affected by a modifier that causes the walls to be full of zits!



Sacrifice 20, 50 health (items unlocked this way span across pages 1 and 2)
Unlocks:
Morphine (row 5, col 8) (disease) (50 health sacrificed)
Swab (row 5, col 5) (page 2) (20 health sacrificed)

To sacrifice health, use the smiling face in the corner of a shop or sometimes in secret rooms.
Simply approach it and it will show its spikes for the you to sacrifice your health on. In shops, this will reroll the items in the shop, while other faces may give mints.

Stuck in some teeth
Unlocks:
Popcorn (row 2, col 10) (disease)

After defeating the final boss and entering through the opening at the top of the room, you will see a mouth. There is something stuck between a pair of teeth. Simply click on it rapidly until it falls off to unlock it.

Cure multiple diseases at once
Unlocks:
Surgery (row 2, col 15)

Rarely, the cure doctor that normally appears on most floors may appear in a special room. In that special room, he will offer to cure diseases at a "discount" (no mints). This will also cure multiple diseases at once and unlock Surgery.



Kill an antibody
Unlocks:
Low Immunity (disease) (row 3, col 14)

An antibody is tough pink enemy that can appear randomly on any floor, though they spawn more commonly later on. In my experience, they tend to spawn more frequently if you have taken a lot of diseases. Simply kill one to unlock Low Immunity.



Tooth brush tooth paste and floss
Unlocks:
Electric Tooth Brush (row 4, col 7)

Simply have the Tooth brush, Tooth Paste, and Floss items all at once (all are unlocked by default).


Unlock combo system
Unlocks:
Combo Bomb (row 5, col 2)

The combo system is unlocked upon beating the game. Very easy unlock.

Rocket Tooth (row 4, col 10) (at one redemption)
Sore Gums (row 5, col 11) (disease) (at two redemptions)

Both of these items are unlocked by filling all of Gob's gums with teeth and redeeming them all for a reward, twice.

Bring the shop keeper shiny
Unlocks:
Sale (row 2, col 6)
Mr Morph (row 3, col 8)
Travelling Merchant (row 3, col 9)

Give away your fillings to the shopkeeper whenever you can, and these items will progressively unlock as they get more fillings. You should aim to give the shopkeeper 999 fillings (the max), as it's required to unlock the Filling gummy. Fillings are primarily found in X dig spots, or by the defeating the Filling Enemy, which always drops a few fillings. The Filling enemy can be commonly found in special rooms.

Free two lab experiments in one run
Unlocks:
Rubber Glove(row 3, col, 10)

Similar Lab experiments are large green glass containers sometimes found in special rooms, holding a pink ball tentacle monster inside. Break two of these in a single run to unlock Rubber Glove.


Get a combo of 200
Unlocks:
X-ray Glasses (row 4, col 1) (disease)

The combo meter is increased over time with speedy, good play. The number at the bottom right of the screen indicates the combo you're currently on. Play fast and avoid getting hit to reach 200 combo to unlock the X-ray Glasses. There are other combo related unlocks, but this is the highest combo needed to unlock them all.


Make 10 steal attempts in a run
Unlocks:
Thief Mask (row 4, col 10)

To make a steal attempt, first find a shop When you're on top of an item, hold 'E' and a small animation will pop up showing you attempting to grab the item. Once the hand touches the item, let go. It will either fail to steal the item or succeed. Both fails and successful item steals contribute to this unlock, simply make all attempts on the same run.

Boop an enemy 10, 30, 60 times
Unlocks:
Cold Feet (row 1, col 5) (at 60 boops)
Lymph Explodes (row 1, col 6) (at 30 boops)
Love Tap (row 1, col 7) (at 10 boops)

Booping is a niche mechanic in the game. To boop an enemy, initiate a wall run, when you dismount, hover the dismount arrow over a nearby enemy. This will 'boop' the enemy, doing small damage to them. This can be easily farmed by booping simple enemies on floor 1 and restarting. Simply do this 60 times across many enemies and runs and you will unlock all boop related items.

Bestiary
The Bestiary is a collection of every enemy in the game. To unlock an enemy's bestiary entry, you will need to collect their bestiary polaroid.

In order to achieve 100% completion, all bestiary entries must be unlocked.

Whenever an enemy is killed, there is a small chance for them to drop their bestiary polaroid. Once its collected, that specific enemy's' bestiary polaroid will never drop again.

Note: Bestiary polaroids despawn after leaving the room it dropped in, be cautious and scan the room carefully before leaving it.

Bestiary Farming
Chances are, common enemies, like bats, will drop their bestiary polaroid much sooner than other, more rarer enemies.

Modifiers that make the game more difficult, such as Hard Mode, or Shorter Run mode, increase the chance of bestiary drops. Conversely, modifiers that make the game easier, such as Busting of Isaac, decrease the chance of a bestiary polaroid dropping.

The effects of these modifiers can be stacked, so multiple hard modifiers turned on will increase your bestiary drop chance significantly, and vice versa.
The bestiary chance from a hard modifier and an easy modifier turned on at the same time will cancel out.

Aside from that, there is no way to influence the chance of an enemy dropping their polaroid.

Except there is...

Metal Gum Flacid
Metal Gum Flacid (MGF) is a secret boss earlier discussed in this guide
(see Secret Bosses (Spoilers)).

MGF is a secret boss sometimes found in special rooms. Whenever MGF is defeated, it will always drop a bestiary polaroid of a regular enemy (NOT bosses nor minibosses). Because of this, MGF is by far the best and easiest way to farm enemy polaroids.

There are currently 65 enemies in the game, but chances are you'll find most of the common enemy bestiary entries, meaning you won't need to farm MGF as much. Once all enemy entries are collected, he will no longer spawn any enemy polaroids.

Nevertheless, farming MGF can still be a hassle, as it's not guaranteed that he spawns on every floor. It's recommended to fill Gob's teeth until he gives the "Increased loot rooms" buffs, which will significantly increase the amount of special rooms generated on each floor and thus, increasing the chance for MGF to spawn.

Bosses
As mentioned earlier, MGF does not drop the bestiary polaroids for both minibosses and bosses. This essentially means you'll have to hunt down the bosses and farm them until they drop it.

Bestiary drops for bosses and minibosses are guaranteed to drop after being killed 10 times (except for Diseaple). You can check the amount of times you've killed a specific boss/miniboss on the bestiary page.

Luckily, the bestiary pages are organized in such a way that orders the bosses and minibosses by the area they appear in.

Tip: Shorter run mode is very beneficial for boss farming, especially for the lategame bosses.

Tip: Rarely, some Goop Canal bosses appear in specific Ortho rooms.

Note: MGF still has it's own bestiary polaroid, meaning they must be farmed for it.

Alt Forms (Bestiary cont.)
Another component of the Bestiary are alt forms. If an enemy has it's alt form unlocked, they have a chance to spawn as that form. Alt enemies have different bullet/attack patters and are typically more difficult than their regular form.

Tip: An enemy in it's alt form is defined by a purple outline.
Note: Bosses and minibosses do not have alt forms, requiring only their polaroid drop for completion.

When hovering over an enemy's bestiary entry, you may noticed a strange "Kills: x" bar under their sprite. This marks the amount of times that specific enemy has been slain. Typically, common enemies will have a much higher kill requirement than rarer enemies.

If you reach enough kills to unlock the alt form of an enemy, the following will happen:
Upon death or winning a run, you will see a message on screen saying "The cavities have formed deeper and darker fissures..." with the sprite of the enemy you've killed enough times in the center of the screen.

This indicates that the alt form of that enemy was unlocked, and it will be reflected in the bestiary by a purple outline around their entry and sprite.

Note: The amount of times you've killed that enemy does not have anything to do with unlocking their polaroid; you can unlock any enemy's polaroid without getting the required amount of kills for their alt form.

Rare Enemies
Most enemies in the game can usually be found in later floors and special rooms. However, some enemies are exclusive only to Ortho while also being rare, making them hard to farm efficiently.

This was a really great guide! I never would've figured out the popcorn unlock, or even known about it. One small critique: You might want to mention that the Tuum garden is unlocked after beating the the final (non schmup) boss once.
