The game features two endings, which depend on the final choice. You can either destroy the damaged crystal (Ending #2) or keep the demon alive (Ending #1). Note that this choice is timed:



You can load the last autosave after completing the game and replay the last section.

Wipe your glasses 13 times.



This achievement can be obtained at the beginning of the game, as soon as it starts raining:





Let Me In, LET ME INNNN!
Try to open a blocked door five times.



Try to open a locked door five times. The counter resets if you try to open different doors.



Attentive Listener
Play with/Listen to the radio for more than 2 minutes.



There are multiple radios in the game. The first one can be found in Andersons' House:

Die for the first time.

Die 10 times.

View to a Kill
Kill one Shadow entity.


Serial Killer
Kill 5 Shadow entities.


Dark Destroyer
Kill 10 Shadow entities.

Kill 20 Shadow entities.

The Reaper's Scythe
Get killed by the hunter.



Get killed by the Hunter, which appears in the village after you leave the Post Office:

Missable

Peeping Tom
Get caught looking at Elizabeth in Children's room.



After picking up the ripped piece of the painting in Room #4, look through the hole on the left:



You may have to get caught and killed by Elizabeth afterward.



Back To The Past
Don't get caught by Elizabeth in Abyssal Dimension of the Rhodes House.



Leave the Abyssal Dimension (after rotating the cross in the cellar) without getting killed. Taking damage is fine. You can load the last autosave and try again if you die.



First Timer
Solve the room 8 puzzle without resetting the mechanism.



Solve the puzzle in front of Room #8 without exiting:



Solution: Press WASD based on the red dots on the right; from top to bottom, left to right.



Getaway Expert
Reach the Post Office without taking any damage from Shadow entities.



After leaving Rhodes' House, reach the Post Office without taking any damage. You can load the last autosave and try again if you receive any damage.



Subconscious Subterfuge
Avoid getting caught by Elizabeth in her subconscious.



Don't get killed in Elizabeth's subconscious. Taking damage is fine. You can load the last autosave and try again if you die.



Surviving The Twins
Avoid getting killed by the twins.



Don't get killed by the twins in Adams' House. Taking damage is fine. You can load the last autosave and try again if you die.



Swerve The Sisters
Avoid getting killed by the Sister's abomination form.



Don't get killed in the catacomb mazes. Taking damage is fine. You can load the last autosave and try again if you die.



Smooth Operator
Avoid triggering the Hunter by using stealth approach.



Complete the final section of the game without getting detected. You can load the last autosave and try again if you get caught.

Collectibles

Free at Last
Destroy all interdimensional blockades.


Genus Genius
Complete all family tree portraits and names.


Illuminati
Light all the candles with the lighter.


Totum Totaller
Find all golden totems.


Dear Diary
Find all missing diary pages.

I triggered Smooth Operator on my first try. I was spotted after breaking the second-to-last crystal, then sprinted to the final crystal and shattered it before the wraiths summoned the hunter and it popped. No idea, man!

A neat trick, if you load a save game where you are in the post office you can trigger one of the achievements.

Beat it, got em all thanks for the guide! Went back and started a new game. Speed run right up till the point you go back down to the basement (to turn the cross) and that is where Back to the Past pinged. Had trouble with Smooth but somehow it pinged on a time where I thought I had been seen by the 4th AND 5th spectre... I think as long as they on't do the animation it is ok but I ALSO think the point at which you need to start crouching is CRAZY early. On my final try (which worked) I started crouching before I even went through the horned door after defeating the twins. So Better safe than sorry for this cheevo I guess?!

Thanks Hanni, I thought there was something weird going on! I know she killed me once so I tried reloading as suggested in the guide but I'm not sure how that works exactly. I reloaded a much earlier game (the automated from picking up the cross) amd played from there all the way up to reaching the post office without being so much as harmed by anything (I got getaway expert) but it still didn't unlock for me.

I left the game while in the subconscious last night as I have no idea what the rules are for Elizabeth in this section. Hard to avoid her without knowing how she works!

@Munty That should have worked. The section starts after rotating the cross in the cellar - and ends (as fas a as I remember) as soon as you rotate the cross again. About Smooth Operator: it's at the very end of the game. The achievements are listed chronologically. As @Norbody mentioned, you have to be very careful. Do not get detected at all in that section.

Hi, struggling to get Back to the Past. I have completed the first time in the house during Abyssal Dimension without getting seen or hurt by hollow Elizabeth let alone killed. Should that not trigger the achievement? Is there another section later in the game that this refers to? I wonder if I'm just trying to get it at the wrong point!

Also similar question for smooth operator. Looking at other comments it seems like this is further into the game than his initial appearance at the post office yes? So does it matter if he sees or kills me during the post office escape?

Smooth Operator bugged. I have completed Smooth Operator 8 times without detecting anyone, and still have not unlocked the achievement.

Unable to unlock Smooth Operator too. I succeeded in breaking all of the crystals and then go killing the villain without being detected, so without the Hunter being summoned, still no achievment unlocked. I wasn't crouched all the way along though. Is this really mandatory? I found it SO long to do it the way I've done it, so do all this while staying crouched, damned, the patience needed!

I have completed Smooth Operator 3 times now without detecting anyone, and still have not unlocked the achievement. I think it is probably bugged. :/

For Smooth Operator, you have to stay crouched the entire time. The trigger is whether you get detected (not whether the monster spawns.) There doesn't seem to be a timer and each time you destroy one of the crystals one of the hooded figures will go to it, so their movements are very predictable. For the last two crystals you have to move out of the way immediately (For the last one, to the right of the arch and up the stairs).
