Full list of the achievements and at which stage you can unlock them.
Detailed info for the harder ones.

Introduction

Most of the achievements unlock easily by just completing the game and finding all the gags, it takes about 6 - 8 hours.

You will find down below each stage-chapter and what achievements you can unlock there, that and the picture of the achievement should be enough info in most cases to unlock each achievement to get the desired 100%.
Some are not so clear and for these achievements i wrote some info as spoiler text.
Let me know if you want me to add more info to a specific achievement.

I recommend to play the game until you have found all gags and then go for a clean up, after playing through all stages you can use "Endless Mode" to play through each of the rounds where you haven't got 100% yet, so you don't have to go through each round individually if you missed something.
But however you wanna do it, this guide will help you.

At the end of the guide i posted a youtube video to a pretty useful walkthrough, use it for extra guidance if you are stuck on a stage.

At episode 3-3, after the spaceship crashes, you have to drag 1 crate all the way to the right to the tank and after the lift takes it down, instead of entering the tank you have to go all the way back to the left where you started.

It's actually quicker this way too.
When you're underwater just repeatedly click on the pipe flowing against you until you're through, it will take a while.

Additional Info:

This achievement requires you to 100% all levels from 1-A to 4-C and 100% all of Steve.

In "Endless Mode", you'll play through each of the rounds where you haven't got 100% yet, so you don't have to go through each round individually.

If you already have 100% on everything and it didn't pop yet, 
go to "Chapter Select" -> "Extras" -> "Steve".

I can confirm that you have to play Steve levels if the 100% gag achievement didn't pop.

@Mado4SXT no the DLCs are not necessary.
If you select any level like e.g. 1-B, you should see a percentage and if it's not 100% you should be able to identify the level where one or more gags are missing, if i am not mistaken that is.

I also added some additional info I found about this achievement:

This achievement requires you to 100% all levels from 1-A to 4-C and 100% all of steve.
