Character choice 1: What did you do when your father was at your door?
1) Pray to a God - Pick one of the Gods and gain affinity to that God.
2) Stand up to him - Gain [+25 max hp]

Character choice 2: What weapon did you specialise in?
1) Pistols - Gain [lugr pistol] and 10 [9mm bullets]
2) Rifles - Gain [rifle .303 MK1] and 4 [.303 rounds]
3) Trench guns - Gain [12 gauge trench gun] and 3 [12 gauge shells]

Character choice 3: You were given a suicidal mission what did you do?
1) Take the mission - Pistols gain [gunslinger], rifles gain [marksmanship], trench gun gains [executioner], gain [heroin] and the debuff [heroin addiction]
2) Abandon the sinking ship - Gain nothing but avoid the [heroin addiction] debuff.

1) Medical goods - Gain 2 [blue vials] and 1 [cloth fragment].
2) Ammo - Pistol users gain 4 [9mm bullets], rifle users gain 3 [.303 rounds], trench gun users gain 2 [12 gauge shells]
3) Food - Gain 2 [dried meats] and 1 [mouldy bread]

Skipping history gives the following:
Choice 1) Gain no HP bonus or affinity.
Choice 2) Gain [lugr pistol] & 10 [9mm bullets]

1) Check Tents - Info then go back
2) Enter the dungeon - Move to next area, inside the dungeon
3) Enter side passage - Info then go back

Inside the dungeon of Fear & Hunger.
1) Check kitchen - Go to Kitchen area
2) Check room in front - Go to Rondon flag office
3) Check the door - Info then go back
4) Go back outside - Info then go back

Kitchen area.
1) Check cupboard - Find [old wheat flour]
2) Check barrel - Nothing

Rondon flag office.
1) Wood desk - Find [small key]
2) Bookshelf - Find [wine vial]
3) Desk - Nothing

After kitchen or Rondon flag office you will hear a guard and have to deal with it
1) Hide in Kitchen - You bypass the guard
2) Hide in room in front of you - You bypass the guard
3) Wait and see - You fight the guard
4) Run outside - You hesitate to leave but still bypass the guard

Fighting the guard.
1) Attack with staff - Coin toss: Pass: You damage it but it keeps coming, Fail: You get hit and are hurt bad but survive
2) Cast hurting - Coin toss: Pass: You damage it but it keeps coming, Fail: You get hit and are hurt bad but survive
3) Evade - You bypass the guard

The foyer.
1) Lavatory - Go to lavatory area
2) Dark corridor - Go to Dark corridor near torture chamber area
3) Stairway - Go to Alter of ALLL-MER area

Lavatory.
1) Investigate - Info
2) Jump in - Game Over
3) Turn back - Return to foyer

Dark corridor near torture chamber.
1) Check chest - Need [small key] if you use it then you get 2 [bear traps]
2) Check corridor - Go to end of corridor & prison cells
3) Go back to foyer - Return to foyer area

End of corridor & prison cells.
1) Check cell doors - Info (If you have the [prison keys] and try and look into the cells you will Game Over)
2) Check door - Need [prison keys] to leave this area. If have then go to Gore hallway
3) Return to torture chamber. (If you have the [prison keys] and try and go back you will Game Over)

Alter of ALLL-MER area.
1) Go back to foyer - Return to foyer area
2) Check altar - Info
3) Check side path - Go to Library area

Library area.
1) Search the bookshelves - Character choice 4.
2) Check the doorway - Go to small area with dead man
3) Return to foyer - Go to foyer area.

Character choice 4: What book did you take from the dungeon of Fear & Hunger?
1) Skin bible of Gro-Goroth - Gain [skin bible of Gro-Goroth] book
2) Skin bible of Rher - Gain [skin bible of Rher] book
3) Skin bible of Sylvian - Gain [skin bible of Sylvian] book

Small area with dead man.
1) Search desk - Decision to force open desk or leave. Force open gain you [lucky coin] then you gain [prison keys]
2) Search body - Gain [tobacco], [blue vial] and [prison keys]
3) Go back to library - Return to library area.

After forcing open desk or searching the body you will fight the ghoul.

Ghoul fight.
1) Hit with staff - Coin toss: Pass: You hit it but it takes your staff, Fail: Injured but still alive. Return to foyer area.
2) Cast hurting - Coin toss: Pass: You damage it but it keeps coming, Fail: Injured but still alive. Return to foyer area.
3) Ignore it and run - Bypass ghoul fight. Return to foyer area.

Gore hallway.
1) Stairs up - Info
2) Stairs down - Go to dungeon area.
3) Examine contraption - Must have [rope] to go to bottom of ravine area.
4) Examine bodies - Info

Dungeon area.
1) Check cells - Info.
2) Walk over the bridge - Go to past the dungeon bridge area.
3) Examine the ravine - Info.

Past the dungeon bridge.
1) Check cells - Info
2) Go to corridor - Go to intersection area
3) Return to other side - Go to dungeon area.
4) Examine ravine - Info

Intersection.
1) Check back on chasm - Go back to past the dungeon bridge area.
2) Check small room - Go to the officer area.
3) Head deeper into darkness - Go to the hanged man area.

Officer area. (Warning if you use 3 actions here before leaving you will be attacked by the Crow Mauler and Game Over)
1) Check weapons - Chose between the [officer sword] or [iron spear] weapons.
2) Check desk - Gain [pipe] and [tobacco]
3) Check chest - Must have [small key], if so then you gain [thorned ring]
4) Check lever - Does nothing.

Hanged man area.
1) Examine ravine - Info.
2) Pull up corpse - Gain [rope], Examine body - Info
3) Head back to the intersection - go to intersection area.

Intersection after getting the [rope].
1) Go to small room - Leads back to gore hallway
2) Go check cells - Fight Crow Mauler.
3) Go back to ravine. Returns to hanged man area

Fight Crow Mauler
1) Swing weapon - Coin toss, Pass: live, Fail: Game Over
2) Cast hurt - Coin toss, Pass: live, Fail: Game Over
3) Slip by - Avoid Crow Mauler

Bottom of ravine area.
1) Climb up - Go to gore hallway
2) Examine wreckage - Info
3) Check camp - Go to camp area
4) Head into the darkness - Go to in the darkness area.

In the darkness area.
1) Keep walking - Go to dead end of cliff
2) Go back - Go to bottom of ravine area

Dead end of cliff.
1) Head back - Go to bottom of ravine area
2) Continue along wall - Go to bottom of ravine area

Camp area.
1) Go back to elevator wreckage - Go bottom of ravine area
2) Check tents - Gain 5 [matches], 1 [ornamental lantern] Check 3x to gain 1 [small thing's amulet]

Dig site area.

I have no idea how I missed the +25hp bonus for getting through the dungeon without getting hit. I was probably just to focused on going back and doing it again to map it out fully.
Anyway I have checked out everything in game that you wrote and updated the guide for O'saa after confirming it.
Had to cut some words down since the O'saa section is over the character limit now.

ah addenum: hiding from first monster also works. It seems as long you don't take any damage during text adventure, O'saa starts with extra 25 hp max cap.

something I noticed:

End of corridor & prison cells.
1) Check cell doors - Info

If you already have prison key, you will die if you do this because you are being chased by the monster.

Also after finding the rope, if you check out cells, you get another crow mauler encounter(though this time you can escape)

But here is biggest thing I noticed: If you avoid taking any damage from archeologist monster or ghoul, O'saa starts with max hp cap of 125 instead.(you don't even need to fight them, just not lose the coin flip, but I think you need to encounter both of them maybe? haven't tested all variations)

Funnily enough, skipping Levi's backstory gives him the best loadout imo, one that isn't possible normally. It gives him a pistol and gunslinger, but no heroin addiction. Rad. His heroin addiction debuff would be cool if it did something other than make you completely useless.
