All achievements can be earned on any difficulty.

Torments

Three of Pentacles
Achieved the top result in a Torment


Doing these on the earlier stages is easier

Heavy Metal Is the Law
Completed a Torment


No Rest for the Wicked
Completed all Torments


No need to get the highest results in all of them, simply finishing them once is enough.

Miscellaneous achievements

This Pounding Heart
Reached Hit Streak: 10


This Pounding Heart II
Reached Hit Streak: 20


This Pounding Heart III
Reached Hit Streak: 35


This Pounding Heart IV
Reached Hit Streak: 50



The Empress
Killed 25 enemies while maintaining a Hit Streak

If all of these didn't unlock passively through your playthrough, run through the earlier stages with Streak Guardian: III equipped. Just make sure to avoid getting hit and making sure you don't miss a beat.

Piece of My Heart
Died in a Chaos Crystal explosion


Get yourself to low health and blow up a Chaos Crystal as you're near it.
Both The Giantslayer Torment and Death's Edge: III has several Choas Crystals you can use right away.

Kill Your Demons
Multi-killed 2 enemies


The Ferrywoman
Killed 8 enemies in 4 seconds


Throughout the game there will be lot's of Chaos Crystals surrounded by Marionettes, you can use these to get both of these achievements.
Otherwise, using the Vulcan can easily yield lot's of quick kills on Marionettes.

Not Shaken, Nor Stirred
Never hit off-beat in a Hell


If you don't get this your first playthrough, I would recommend going back to an earlier stage and adjusting the difficulty.
There is no need to hit every beat and not all beats need to be perfect beats, you also don't have to hit beats when you swap weapons, jump or move.
The achievement only voids specifically when you hit an actions like shoot, your ultimate, fast-reload or dash off-beat.
Hitting the fast-reload is optional and you can optionally skip the fast-reload to maintain a perfect run.

Raining Blood
Killed two Seraphs in one attack


If you don't get this on on your first playthrough, best odds are playing a level where 2 Seraphs spawn on the lowest difficulty (the final encounter on Nihil will have one) using your Vulcan's Ultimate to pull together the Seraphs and using the Persephone (Shotgun) ultimate to kill both.
Alternative, 2 Seraphs will spawn on the first encounter on Nihil on the highest difficulty.

Pazifist
Cleared a Hell using only Paz


Performing slaughters and kills by Chaos Crystals are allowed.
This achievement only voids if you kill an enemy with any other weapon then Paz.
Recommended on an earlier level on an easier difficulty if you are struggling.

The Sword is Sharper
Killed a Judge's Aspect with Terminus


No need to do the entire fight with the Terminus, it's only the last hit that counts.

Most achievements listed here are recommended to do last since these are all cumulative across all runs.

Queen of the Underworld
Killed 50 Behemoths


If you want to farm this, a guaranteed Behemoth spawns in Slaughter Mastery: III

Soaring in the Deep
Soared (Dash + jump) 100 times


Will unlock as soon as you get the required 100.
Make sure to hit the inputs on the beat.

Highway to Hell
Got 666 kills (game total)


Just kill stuff, hard to miss if you get all other achievements.

Who Wants to Live Forever
Died 20 times


If you somehow don't get this after getting all other achievements (good job!), you can farm this on a Torment on the highest difficulty.

For those who aim to do 'Not Shaken, Nor Stirred' as intended, adjusting the colour of 'good' hit indicators in accessibility settings to something more noticeable can give a better idea when you have to start over. Also, it seems fast reload misses do not count.

@Ithildin How'd you manage that? I set mine to 500ms for 120bpm and it kept falling off the beat. I changed it lower or higher on 1ms increments and the closest I got was 502ms which still went off beat after 30 seconds or so.

Very stupid that this works but I just tested it and it does:

To get Not Shaken, Not Stirred
1. Go to Settings > Accessibility
2. Enable Rhythm Assist Mode (Always on beat), at the bottom of the Accessibility page
3. Play through a level on any difficulty, doesn't matter if you stay on beat or not

Pretty sure Raining Blood is bugged. I've killed 2 seraphs with the hounds ultimate like 10 times and nothing happens

I did use a macro on my mouse to complete Shaken not stirred cause I couldnt be bothered with actually hitting on beat. yes its a cheese but I dont care

i did all the torments on different dificulties while trying to beat the game, no achiev
just redid ALL OF THEM ONCE MORE ON BEAST, GOT 3 SKULLS >NOTHING<
